Well its nearly end of the 4th quarter for some so most are renewing their health coverage. We got a form we fill out here every 3 months ( quarterly). Some folks get a great health package if the report comes back in their favor. So its a win win for those that are healthy and active vs couch potato chip eating bone head.
OP, Copy the form (assuming it has company letterhead or a signature with title) and present it to DOL, who will tell you if it should be presented as a potentially discriminatory or illegal inquiry to the Human Rights Commission or EEOC.
Who are you returning the forms to, HR or the health insurance carrier or other? The company my sister works for did something like that. If they were overweight and participated in the program and went to the gym, a chain kind, they got credit and slight discount on health insurance. They got a discount if they didn't smoke, and some other program. But it was kept confidential from HR and run by a outside agency.

If they are proving you with subsidized health insurance they can ask anything they want, even require a physical. I had to take one once for a job, after accepting an offer. We do a wellness program online that benefits us with either $1,000 toward an HSA or reduced premiums, and it requires a very comprehensive assessment, including blood pressure, cholesterol and blood sugar levels. Neither your boss nor co-workers have access to that data. A third party manages it.
