So I had a final interview for a job and I didn't mention that I have a planned and already paid for vacation about 2 weeks after the start date will be. I've read after the fact that this should be mentioned during the interview but I didn't do that. Now I'm wondering if I get the job offer, how I bring this up and if this can be saved or am I doomed?

Since the interviews just finished and there's no offers it's probably their anticipated (hopeful) start date. You can probably just work with HR to have it pushed back since these start dates are tentative due to waiting on background checks, etc...

How can you go to an interview and not bring that up knowing full well you had this in the works?

Because OP apparently isn't as perfect as you are.

OP - wait until you get a formal offer. When you receive the offer, contact the HR person (or hiring manager) who did the final interview letting them know about your planned vacation. Most companies will be flexible with respect to this as long as you let them know up front.

Interviews are not the time to be talking about planned vacations unless it is mentioned casually.

^^^^ Agree 100%. To me, it's not too late. Bring it up when/if you get the job offer. As a hiring manager, I wouldn't be perturbed unless you brought it up after the deal was made.

IMO, it's not up to the interviewee to bring this up during interviews. The interviewer should ask about any planned vacations.

Since the interviewer didn't ask, OP should tell them about it when the offer is received.

Oh ok. Maybe they have a firm date to start training classes or something. In which case a vacation in the beginning could be problematic.

Then it's a very possible problem. I'd reach out to the recruiter or Hiring Manager. If you weren't a top candidate for this class, you're likely going to be SOL, but might have a chance for their next class.
