I don't think you'll have too much to worry about on the islands with regard to risks of contracting diseases. Mozzies who are carriers seem to be a bit fussy on where to lay eggs. That said, a repellent is good to use just to help ward off the little buggers. They do seem to enjoy snacking on "foreign food". Fans and air conditioners can help keep them at bay at night, as well as help keep cool at night. A good sunscreen would be a good idea to take along.

I added in the Similans just to note that there are several islands in Thailand that are scenic and outstanding. On the Gulf side, and farther north, there are also a few islands that are impressive, such as Koh Tao (Turtle Island).

I think in terms of sheer uniqueness Hawaii is hard to beat. It has so many landscapes from rainforest, to scrubland, to true desert (with cacti) to of course the towering peaks of the volcanoes on the big island. What other tropical island that size gets snow? The Big island is actually the second highest island in the world, after New Guinea, which is the second biggest and highest island in the world (which I should have also mentioned). The Maldives etc are really nice, but there's nothing there that you can't find in dozens of other isles.
