So far only India. I had to send my passport with some forms and small photos to the nearest consulate 370 km away. I think India is in the process of making a VOA system for many countries now, though.

China, although I needed a letter of approval before getting a VOA in Vietnam.

China and Russia.

Got the Chinese visa in Hong Kong from a rather dodgy looking visa agency. To my surprise, everything was fine. The Austrian Consulate recommended him, because the official visa office handled Austrian applications a bit slow at that time. Our Chancellor and Foreign Minister both met the Dalai Lama in 2012, so the Chinese government was a bit pissed at that time.
