When you don't understand what someone said: ㄏㄚˊ, as in haaaa? with a little bit of nasal sound.
It can also be used when one's disappointed, but the tone would be different.

At the end of an affirmation: Can't think of anything now.

It's the same thing. In England it's usually written 'er', because the R is mute.

I think in many places in the US, Americans may use both depending on the context. I know in California, we use "uh" when we are unsure of something when in mid sentence. We may use "huh" when asking a question like; "It's kinda cold huh?". Sometimes we use "eh" when it's more of a light-hearted nature like; "Guess she told you eh?". We may still use "huh" in that situation too. If we're confused, we always use "huh?"
