Obviously, Vatican City. But despite its misnomer, it has few if any amenities one associates with a city.

Geneva is a good nominee. Pristina, the capital of Kosovo, is about the same size.

San Marino City. Also a capital...

Geneva population is about 500,000. - not a small city.
Monaco ~37,000

Second Luxembourg City, about 120k people. Mentioned often. Probably due to its significance with various European organizations.

By major are we talking 'well known'? In the UK perhaps Oxford fits the bill? Thanks to the world renowned college and its scientific importance? Population is only 150,000 or so.

Macau might be a good contender. Decent name recognition, physically quite small at about 13 square miles of land though it's quite dense. That density gives Macau a real city feel to it. It's also not quite part of a larger metropolitan area due to internal border controls of sorts.

Even smaller Monaco of which Monte Carlo is a quartier might also make sense.
