      Update employee health reporting policy to reflect new code changes. Forms 1a, 1b freom Annex 7 located in rear of code after index may be used as a tool. Person in charge could not locate policy.

      Excessive grese build-up in city sanitary servive. Your establishment is one of two on effected sewer line. You were notified in spring to get on a regular grease trap maintenance program. At time of inspection documentation of maintenance of grease trap could not be provided. Health services Department will follow-up to verify grease trap obtains regular service as needed.
