      Part of ceiling missing in back room. Plywood has been removed as roof was leaking. Strong smell of mold in back room. Need to fix leak and install a cleanable ceiling other than plywood.

      SIdes of three tank sink need enclosed.

      Walls in back area have holes or gaps making cleaning harder. FLoor has been replaced in the back. All floors must be smooth and easily cleanable.

      Chemicals are being stored on the top shelf in the back above food. Move this so it is on the bottom shelf where cross-contamination cannot occur.

      Floor needs fixed where tiles missing. Area by three tank sink needs enclosed, and there is to be no food storage in that area.

      Roof leaks badly in back. You have 90 days to correct this.

      Back area needs clean once roof is fixed. Mold is an issue.
