      The employee restroom in the boys restroom area CONTINUES to not hava a self closing door. Toilet room doors shall be provided with self closing tight fitting doors. THIS HAS BEEN A CONTINUED AND REPEATED VIOLATION FOR NUMEROUS INSPECTIONS. INSTALL A SELF CLOSING DOOR TO THE EMPLOYEE RESTROOM.

      The employee restroom door was open. THIS CONTINUES TO BE A REPEAT VIOLATION. Toilet room doors shall remain closed except during periods of maintenance and cleaning. Install a self closure which will keep the restroom door shut. THE RESTROOM DOOR HAS A SIGN POSTED DO NOT SHUT THE DOOR AFTER USING AS IT WILL LOCK AUTOMATICALLY. Suggest replacing the door knob so the door will not automatically lock AND REMOVING THIS SIGN so the restroom door can be kept closed per the STATE OF IOWA FOOD CODE.

      The handsink in the kitchen is 86 degrees and in the employee restroom at 78 degrees. A handwashing sink shall be equipped to provide water at a temperature of at least 100 degrees. Shall correct this ASAP.

      The handsink in the kitchen was being blocked by a large trash can. Handwashing sinks shall be accessible at all times for proper handwashing. CORRECTED ON SITE.
