Otherwise food must be discarded after 4 hours at the present temperature or moved to another cooler if 4 hours has not been exceeded.

The food guard is yet to be installed in the front of the smoothie prep cooler. The guard must properly designed and installed to adequately protect food from consumer contamination. Please instruct employees to keep the ice in the bin at the front counter protected from consumer contamination, or an additional food guard may be required. Adjust the shelving in the warehouse where food and food-contact items will be stored. All bottom shelves must be 6 inches from the floor.

Incorrect test strips provided to verify the concentration of quat sanitizer. Provide quat test strips. Concentration must be between 200-400 ppm.

- Back flow/back siphonage protection (Backflow prevention device requirement not met 5-203.14, p 150, IAC 481-31.1(11))
Blender washer (small, red device) requires that an additional backflow prevention device be provided when it is installed. Please verify with the Cedar Rapids Buliding Dept - Plumbing Inspector as to what device will be required when this item is in use.

Provide a container for transporting ice which was intended to be used with food. Currently using a non-food grade bucket.

The shelving in the warehouse where food and food-contact items will be stored must be sealed to provide a non-absorbant, cleanable surface. Provide at least two coats light-colored paint or varnish on the shelves.
