Mechanical washing/sanitizing (Chemical sanitization (concentration): temp/pH/hardness 4-501.114, p 127) Chlorine solution concentration reading at 10 ppm. Minimum concentration for sanitizing must be between 50-100 ppm. Must have dish machine serviced prior to serving the public and complete mail-in recheck to show the dish machine is reaching the required concentration for proper sanitation.

Minimum concentration for sanitizing must be between 50-100 ppm. 2) Dish machine reaching 118.8 degrees. Label instructions on machine state machine should be washing a minimum 120 degrees. Have dish machine serviced so that the chlorine solution is within acceptable range of concentration and reaching 120 degrees during wash cycle.

Back flow/back siphonage protection (Air gap, design: 1" minimum or 2 X supply inlet, whichever is greater 5-202.13, p 149) 1) Dish machine is directly plumbed. Must have a minimum 1" air gap to prevent back flow siphonage. Contact licensed plumber to provide air gap. 2) The hose attached to the mop sink faucet is laying in the mop sink. Cut hose so that it is a minimum of 1" above the rim of the mop sink to prevent back flow siphonage.
Plumbing: installed/maintained (Backflow prevention device, design standard not met 5-202.14, p 149) Atomspheric vacuum breaker attached to mop sink faucet is leaking. Replace device for back flow prevention.
Equipment adequate to maintain temperature (Cold & hot holding equip, inadequate in number & design 4-301.11, p 117) Reach in cooler is not commercial grade refrigeration unit. Equipment for cooling food must be adequate to maintain temperture of 41 degrees or less. Provide commerical grade refrigerator.
Food contact surfaces (Storage: equipment/utensils, single service & use items 4-903.11, p 141) Bowls in the kitchen cupboard are being stored with the eating surface being exposed. Dishes must be stored inverted. Store bowls inverted.
Non-food contact surfaces (Nonfood-contact surfaces: designed, cleanable 4-202.16, p 107) All non-food contact must be nonabsorbent and cleanable. 1) Must caulk around the coffee counter and all wall junctures. 2) Spoons in kitchen drawer are being stored in cardboard containers. Provide a container that is cleanable and nonabsorbent. 3) Utensils being stored on a bar towel in a drawer in coffee area. Remove towel.
Walls, floors, ceiling, lighting (Floor/wall junctures 6-201.13, p 167)
Smoking area: designated/posted (Smoking prohibitions in public buildings: signs posted, ashtrays present, smoking observed. IAC 641-153) All entrances and exits used by public must have state approved "No smoking" signs posted. Given at time of inspection. Post signs.
