      The stainless steel plate covering the old pop station must be caulked to provided a easily cleanable surface.

      The flooring materials in the newly remodeled areas must be installed so that they are easily cleanable.

      Boxes of produce stored on floor of walk-in cooler. Box of food stored on floor under shelves and box of shredded potatoes on floor by shelves in walk-in freezer. All food shall be stored on shelves at least six inches off floor.
    • Labeling (Food not properly labeled 3-602.11, p 93 IAC 481-31.5)
      Black container of sugar stored on shelf in bakery room not labeled with contents. Need to label containers with contents when food is removed from original package, especially those foods that are white colored.
  • Plumbing/Water/Sewage
    • Accessible/soap & towels provided (Handsink maintained/accessible/restricted use 5-205.11, pg 151)
      The handsink in the meat prep room is not accessible as a large yellow cannister containing breading mixes is in front of sink. Need to keep handsinks easily accessible for use. Manager plans to move breading mix to smaller storage containers and remove large yellow from prep room.
    • Plumbing: installed/maintained (Plumbing maintained in good repair 5-205.15, p 153)
      The area around drain in dishroom has some standing water. Per manager, there is a broken area of tile in the plumbing system of dishroom that will be repaired in near future. The drain in meat prep room was recently repaired and is now draining properly.
  • Facility/Equipment Requirements
    • Food contact surfaces (Food contact surfaces not clean to sight or touch 4-601.11, p 129)
      1) One of rubber spatulas on wall rack is torn and no longer cleanable. Spatula was discarded during inspection. CORRECTED. 2) Food contact surfaces in need of cleaning include: 1) Lower shelf of prep table where cutting boards are stored, 2) Shelf above prep sink in bakery room, 3) Inside surfaces of Traulsen reach-in cooler where bakery items are held, 4) Shelves of dry storage room where there is spillage, 5) Upper racks of reach-in cooler at salad prep area.
    • Food contact surfaces (Utensils not maintained in good repair 4-502.11(A), p 128)
    • Non-food contact surfaces (Wiping cloths, in sanitizing solution, used one purpose 3-304.14, p 66)

      1) Ceiling vents in all restrooms are getting a large amount of dust build-up. 2) Vent system/grills above fry station and grill are getting grease build-up and need to be cleaned.

      Per 2005 Food Code, the Consumer advisory now has a second component: A Disclosure statement identifying the specific foods that are undercooked must be identified in addition to the Reminder statement that is already required. Manager plans to have new placard made to identify which items are offered undercooked and display on make station. Information on new requirement given to manager.

      Need to provide a covered receptacle in employee women's restroom for the disposal of sanitary napkins.

      "No Smoking" sign needs to be posted on entrance to establishment per sign requirement. Sign given to post.

      Raw chicken stored over pizza in refrigerated kitchen cabinet. Corrected during inspection. Food is being stored correctly in the meat cooler.

      Hobart mixer is not being used and is to be removed from the kitchen. Pizza oven remains to be cleaned. Ice cream machine has been cleaned and has been placed on weekly cleaning schedule. Mixer in dessert area needs more frequent cleaning.

      Raw chicken and beef are being stored above green beans and frozen pizza in the refrigerator cabinet in the kitchen. A bin of raw chicken is stored over beef and eggs in the walkin cooler. Raw animal foods shall be stored in such a way as to prevent cross contmination of other foods. Corrected during inspection.

    • Accessible/soap & towels provided (Handsink maintained/accessible/restricted use 5-205.11, pg 151)
      A dish was found in one of the handwashing sinks in the kitchen. A handwashing sink may not be used for any other purposes.
    • Plumbing: installed/maintained (Plumbing maintained in good repair 5-205.15, p 153)
      The spray hose at the dish sink is leaking and water is dripping and pooling onto surfaces. Need to repair.

      No thermometer was found in the refrigerator cabinet in the kitchen. Thermometers shall be provided and accessible in all refrigerated units.

      The pizza oven, undersurface of the Hobart mixer, both ice cream machines, and the mixer in the dessert area have build-up and need to be cleaned more frequently.

      Provide an air gap of at least one inch at the food prep sinks in the kitchen. The plumbing must be directed to the nearby floor sinks.

      The drain lines from the two ice machines must be sloped to drain. The drains from the ice makers as well as the ice bins must be hard plumbed. The water line for the coffee at the rear server area is leaking and must be repaired. A new toilet is being installed in the men's employee restroom.
