This is a re-check of pest controll. The establishment has had pest controll treat the facility yesterday and today. Floors have been cleaned. There is still standing water between tiles due to lack of grout. Many roaches were observed in the server area, kitchen, and dishroom. The establishment has not met the criteria for reopening.

Establishment is infested with cocroaches. Many live roaches were observed in server area, both sides of wall on main line and the threshold between server line and dish room. There area also many areas in the establishment that area wet and soiled. In the basement, kitchen, server line, and dishroom. Areas of the establishment are under contruction. The establishment is treated twice per week by ABC pest controll. The establishment has agreed to voluntairlly close until pest infestation is under controll and areas identified are cleaned and dry.
