Observation: Boxes of chips stored on the floor. Food shall be stored at least 6" off the floor and covered to prevent contamination. Corrected on site.
- Storage of clean linens, single-service, and single use articles
Observation: Boxes of single use items such as plastic cups were stored in the back storage area on the floor. All single use utensils must be stored at least 6" inches above the floor. Store on shelves.Correctes on site.
- Outer openings are protected
Observation: The delivery back exit door have light showing through the bottom. All outer doors shall be provided with self-closing tight fitting doors to prevent entrance of pests and rodents. Re-install weather-stripping or replace the door.
- Poisonous or toxic material containers
Observation: Antibacterial moisturizers stored next to single use items such as the paper plates and several can food. Poisonous or toxic material must be stored separated so they cannot contaminate food and single-se items, or utensils. Removed and corrected on site. COS

- Restriction-presence and use
Observation: A spray container of "Ortho" insecticide on the premises. Must always have approved insecticide for food establishments. This was discarded during this inspection. CORRECTED ON SITE.
- Drying mops
Observation: One mop was improperly drying.After use, mops shall be placed in a position that allows them to air-dry without soiling walls, EQUIPMENT, or supplies. CORRECTED ON SITE.
- Maintaining premises free of litter and unnecessary equipment
Observation: The facility has a small black apartment type refrigerator which is nonfunctional.The PREMISES shall be free of Items that are unnecessary to the operation or maintenance of the establishment such as EQUIPMENT that is nonfunctional or no longer used. REMOVE THE NONFUNCTIONAL REFRIGERATOR.
- Smoke free air act
Observation: The food establishment had no smoking signs, but NOT the official no smoking sign to be posted at all entrances. Given during this inspection to post. CORRECTED ON SITE.

1. Observed new addition to store that the walls are not painted making them not smooth, cleanable and non-absorbent should they get wet. 2. Cove base is needed in all the rooms of the new addition, if the sheet rock gets wet, will deteriorate and not be smooth, cleanable and non-absorbent. 3. Provide a threshold on floor to employee RR, not flush with the floor and will collect dirt. It is not smooth, cleanable. 4. Lights are not shielded in the storage room that has food (chips) in it. Can use shields on lights or shatter proof bulbs to prevent contamination to food if light shatters. 5. Observed soiled or water damaged ceiling tiles above the cigar room. May have leak in roof. Repair and replace tiles as needed.
