      Few boxes of food are on the floor in the dry storage area and a box of cheese is on the floor in the walkin cooler. All food needs to be stored at least 6 inches off the floor.
    • Labeling (Food working containers not identified with common name of food 3-302.12, p 63)
      A crock of white powder in the kitchen and a container of ? breading mix in the dry storage room are not labeled. All food, not easily identifiable, that is removed from the original container needs to be labeled with the common name.
  • Plumbing/Water/Sewage
    • Accessible/soap & towels provided (Hand drying not provided 6-301.12, p 173)
      Provide accessible paper towels at the bar handsink.
  • Facility/Equipment Requirements
    • Food contact surfaces (Beverage tubing/cold plates in contact with stored ice 4-204.16, p 110)
      The pop lines are in direct contact with the ice in the ice chest at the bar, allowing for contamination of the ice. Provide a plexiglass (or other) barrier so the lines do not touch the ice.
    • Non-food contact surfaces (Nonfood-contact surfaces exposed to food: cleanable, nonabsorbent 4-101.19, p 104)
    • Walls, floors, ceiling, lighting (Floors, walls, ceilings installed, are not easily cleanable 6-201.11, p 166)
      Floor tiles are missing in the kitchen corner by the pizza oven making the area difficult to clean. Replace the floor tiles so the surface is smooth and more easily cleanable.

      Need to provide quat test strips to check concentration of sanitizer in the kitchen and at the bar.

    • Handwashing facilities (Handwashing signs not posted 6-301.14, pg 173)
      Need to post a sign at the handwashing sinks that notifies employees they must wash their hands.
    • Accessible/soap & towels provided (Hand drying not provided 6-301.12, p 173)
      Need to provide paper towels at the bar handwashing sink.

    • Food contact surfaces (Food contact with unclean surfaces or probe type price tags 3-304.11, p 65)
      Beverage tubing is in contact with the ice in the bin at the bar. Need to provide a plexiglass shield or other type of barrier so the ice does not touch the pop lines. Noted on last year's inspection also. The 2 chest freezers in the kitchen are in disrepair and need to be replaced. Owner states she is planning to do this soon. Noted also on last year's inspection.

      No test strips available for checking concentration of Quat sanitizer being used at the bar and in the kitchen. Need to provide test strips so concentration of sanitizer can be checked.
  • Plumbing/Water/Sewage
    • Handwashing facilities (Hand sink hot water supply: 100° F, 15 sec, no cold water 5-202.12, pg 149)
      No hot water at the handwashing sink in the kitchen. Water had been turned off under the sink. Appeared to work when water was turned on. Keep hot water turned on to sink. If not working correctly, make repairs as needed.
    • Accessible/soap & towels provided (Hand drying not provided 6-301.12, p 173)
      Need to have paper towels accessible at the handwashig sink at the bar.
  • Facility/Equipment Requirements
    • Thermometers provided/accurate (Thermometers: provided & accessible 4-302.12(A), pg 120)
      No thermometer found in the refrigerator in the back room. Need to provide accessible thermometers in all refrigerated units.
    • Food contact surfaces (Food contact surfaces not clean to sight or touch 4-601.11, p 129)
      Ice machine has buildup that is dripping into the ice. Need to empty, wash, rinse, and sanitize before refilling. Need to thoroughly clean the ice machine more frequently. The large chest freezer in the kitchen has a bad gasket. Plans to replace the freezer within the month. The ice in the chest at the bar is in contact with the pop lines which are dirty. Need to provide a barrier ( ie - plexiglass) so ice does not touch the pop lines.

      Ice scoop sitting with handle in ice at time of inspection. Discontinue this practice. Store ice scoop with handle protruding from ice to prevent contamination. Bin where flour is stored is made of plastic that is not food grade. Provide a bin for flour made of food grade plastic.

      Clean floor sink located under pizza oven. Clean dough roller thoroughly by taking roller apart and detail cleaning on a regular basis as needed.

      Shield light located above ice machine near walk-in cooler. Either use bulb coated in shatter-resistant material or provide shatter guard for bulb.
