  • KDA_4_60111A - --There were several plastic food grade containers stored as clean on a shelf in the back kitchen area that had sticky label residue on the from a previous use. COS Items were placed in the dish room for cleaning.

  • KDA_5_20515B - --The hand washing sink in the ware washing area, has a leak at the turn off handle.
  • KDA_6_50111 - --In the front women's restroom in stall one, there is a black rubber piece under the base of the toilet that is sticking out. In both of the front restrooms the tile on the baseboard area is cracked in areas. There is various cracks on the flooring through the establishment. In the dry storage area, there is paint peeling on the north wall and the drop down frames have rust on them. At the mop sink, there is paint peeling on the walls. Through out the kitchen, there is missing base boards. In the back men's restroom, there is missing tile under the men's urinal. In the back women's restroom, there is two stained tile. Person in charge said they are replacing the urinal in the near future.
  • KDA_6_50112A - --In the front men's restroom, there is a piece of paper towel stuck to the west wall. There is dirt and debris on the walls also. In the women's front restroom, there is hair and debris on the south wall, there is dirt on the base of the toilets. In both of the front restrooms, there is dust on the ceiling and vents. The ceiling and vents through out the establishments have an accumulation of dust. At the mop sink, there is black mold and dirt and debris on the wall and around the floor area. In the bakery area under the hand washing sink, the base board is black. In the ware washing area, there is a drain with cloudy standing water in it. There is black mold, rust, and dirt debris on the base boards through the kitchen. In the men's back restroom, there is part of a paper towel and standing water in the urinal. In the women's back restroom, there is black debris under the diaper changing station on the wall, the base of the toilets have dirt and debris. The person in charge said they clean the restrooms three to four times a day.
  • KDA_6_50118 - --The hand washing sink in the ware washing area, has an accumulation of dirt and debris. The men's and women's hand washing sink in the front, has an accumulation of black debris in the basin.
  • KDA_7_20111B - --on a shelf in the ware washing area, there was a green bucket with a white powder in it labeled detergent and a bottle labeled bleach. The detergent and bleach was sitting over metal pans. Person in charge said those items should be in the chemical room. COS-detergent and bleach was moved.
  • KDA_3_30211A1b - --There were two plastic food grade containers of raw hamburger used for making hamburger patties that was being stored on a shelf directly above a pan of cooked roast beef and a pan of sliced cheese located in the RIC-2(Mcall). COS Raw hamburger was moved to the shelf below the roast beef and cheese.
  • KDA_3_30212 - --There were two bottles of butter substitute being stored in the gill area that were no marked with a common name on them.. COS Items were marked.
  • KDA_3_50116A1 - --There was a pan of chicken tenders at the buffet-1 that had a internal temperature of 122.2F. PIC said they were only out for about thirty minutes. COS Item was reheated to 180F
  • KDA_3_50116A2 - --There was a plastic pan of cottage cheese at the salad bar with a internal temperature of 45.8F. PIC said it was placed out on the salad bar about thirty minutes ago. The ambient temperature of the salad bar was 41F. COS Item was placed in the RIC and another pan of cottage cheese was placed in the salad bar.
  • KDA_4_20211A2 - --There were several plastic rubber spatula stored in a bucket as clean that were burnt and had pieces of plastic missing around the blade. COS Items were discarded. There were several plastic container stored as clean that had pieces of plastic missing from around the edges of the container. COS Items were discarded.
  • KDA_4_60111A - --There were several plastic container stored as clean on a shelf in the back kitchen area that had sticky label residue on them from a previous use. COS All items were placed in the dish room for cleaning.
  • KDA_5_10311B - --There is no hot water going to the hand sink located in the WIC were they cut meat. PIC said there is a leak in the wall so they have shut the water off going th that sink.
  • KDA_6_30111 - --There was no soap in the dispenser at the hand sink located in the front bakery area. COS Hand soap was placed in the dispenser.
  • KDA_6_50111 - --There are a few cracked floor tiles in the dish room area. The ceiling tiles is peeling above the chicken rotisserie located in the back bakery room.
  • KDA_7_20111B - --There was a bottle of round up weed killer be stored on a food prep table located in the smoke shed. COS Chemical was removed.
  • KDA_Defacto - --All frozen foods are stored frozen. --HH-(buffet-1) roast chicken 138F, meatloaf 147F green beans 160F. HH-(buffet-2) pot roast 168F, liver and onions 154F, lasagna 140F. HH-(buffet-3) Mexican rice 140, taco meat 160F, chili verde 139F. HH-(buffet-4) baked potatoes 166F, cheeseburger 144F. HB -chicken pasta 160F. HB- white gravy 160F --Plate Temperature 172F. Three vat sink was not setup during the inspection. Test strips were provided. --QT- sanitizer bucket- 200ppm --ServSafe --WIC- beef loin 41F, pork ribs 37F, carrots 39F, chocolate milk 30F, cut melon 35F. RIC-1(bev air0 cheese cake 41F. RIC-2(McCall) sliced cheese 41F, roast beef 40F. RIC-3(McCall) pasta 37F, cooked chicken 43F. CH-(salad bar) cole slaw 41F, crab salad 43F, diced eggs 39F, diced ham 40F. CH- pico de gallo 40F, sour cream 40F. CH- milk dispenser 36F
