    This facility is using unsecured rodent bait inside of the building. Rodent bait is poison and may not be used inside of a food facility. Remove all rodent bait from the inside of the facility.
  • In-Use Utensils & Between Use Storage (46.302)
    The tongs for the bagels are dangling on a cord very close to the floor. Place a bin for these tongs at this location.
  • Cold Holding Potentially Hazardous Foods (46.385(a)(2)-(a)(5))
    Some of the sofrito was out of temperature with internal temperatures ranging from 43 to 50 degrees F. All foods in cold holding must be maintained at or below 41 degrees F. The out of temperature product was discarded. The cooler was overfilled. Do not overfill the cooler with product.
  • Cooling, Heating, & Holding Capacities (46.611)
    One of the blower unit fans in the meat room was not operational. Repair this blower unit. Do not leave product stored in the meat room until this repair is complete.
  • Outer Openings Protected, Exterior Walls & Roofs Protective Barrier (46.922(e)(f))
    There are still possible rodent entry points at the two sets of rear doors. Install new door sweeps.

    There were still some mouse dropping observed in the facility. Continue to clean up the droppings on a daily basis. Pests must be controlled in the facility.

    Rodent bait (poison) was again found in an unsecured location under the frozen food cases. Rodent bait must be secured in covered, tamper resistant bait stations. Rodent bait stations may not be used on the inside of a food facility.

    Facility is making progress in controlling the rodent problem. There are significantly less signs of rodent activity in the facility. Continue with efforts to eliminate mice from the facility.

    There has been food damaged by mouse activity in the facility. Discard all damaged foods. Clean all packages that have come into contact with mouse droppings with sanitizer. If the packaging can not be cleaned, the food must be discarded.

    The facility has a mouse problem with droppings observed in isles 2, 3, 5, 6, 7, and 9. Take apart the shelving and cases in these isles to look for nesting sites. Clean up the droppings. Pests must be controlled in the facility. Contact your exterminator. This facility must be on weekly extermination schedule. This facility must provide the Allentown Bureau of Health with extermination invoices for exterminations for the last six months.

    Facility has a mouse infestation which seems to be concentrated in isle two. Take apart the shelving and cases between isles one and two to look for nesting sites. Clean up droppings and remove the five dead mice that were on glue traps in the facility. Pests must be controlled in the facility. Contact your exterminator to help in this process.

    A total of 18 mouse droppings were observed among the snack shelves, the rice shelves, and the bean shelves. This was a significantly less number than observed on 17 June 2011. Continue to clean and sanitize these shelves at least daily as long as the problem persists. Pests must be controlled in the facility.

    There were mouse droppings observed on the snack shelves, the bread shelves, the rice shelves, and the bean shelves. Clean these shelves. The facility is aware of the problem. The exterminator comes once a month, but the facility has also been making emergency calls lately. Control pests in the facility.

    An individual was observed chewing gum in the deli. There may be no eating (or chewing gum) in a food preparation area.

    There were two bags of chicken temporarily stored on the floor in the deli. Food must be stored elevated at least six inches.
  • Equipment in Good Repair & Proper Adjustment (46.671)
    There is a very large gap between the door and the unit when the produce walk-in door is closed. Repair this gap.
  • Plumbing Facilities in Good Repair, Cross Connection Prohibited, Scheduled Inspections, Cleaning Water Reservoirs (46.825(b)-(e))
    There is a large puddle of water next to the meat room walk-in freezer. Find the cause of this leak and repair it.
