The quarternary ammonia concentration in the sanitizing solution of the 3 bay sink was connected to cold water supply only from pre-measured dispensing unit. Temperature at this time was measured at 52*F. Facility instructed that minimum temperature of solution is to be 75*F. Facility is to hand mix solution until pre - measured dispensing unit is corrected. Monitor to assure compliance.

Build up of dirt / dust on blades of ceiling fan in prep area. Correct within 3 days
