Food facility does not have available chlorine sanitizer test strips or test kit to determine appropriate sanitizer concentration. Correct asap.

Person in charge was not aware of any chemical sanitizer being used on equipment and utensils after washing and rinsing them in the 3 bowl sink. Reviewed procedures for 3 bowl sink use (wash, rinse and sanitize using 1 capful or 1 Tablespoon of bleach for EVERY gallon of water) then AIRDRY all dishware (do not towel dry). Dishwashing was not observed

Observed two (2), round cutting boards are completely worn through with large broken holes in cutting surfaces, cutting boards not discarded as required. Indicated worn out cutting boards were discarded immediately.

Observed cooler and freezer unit handles and employee hand wash sink, in kitchen area, with an accumulation of old hard grease, dirt and food residue, debris on handles contact surfaces. Cleaning was conducted during inspection.

Paper towel dispenser empty and no hand soap at the handwash sink in the employee rest room. Corrected at start of inspection.

No hair restraints worn by 3 food handlers.Supply within week.
- Time as a Public Health Control for Potentially Hazardous Foods (46.385(c))
About 8 raw eggs were in container on cart in front of cook line. Surface = 63*F.Cook reports they had been there 1 hr. Moved to cooler. If you want to keep them on the cart, out of refrigeration you need to bring only a small amount out at a time. And to write a note that indicates the time that is 4 hours past the time when you took them out of the refrigerator. For example,you remove them from the refrigerator at 2pm, and only the amount you will use within an hour or less, and you write a note that reads discard eggs at 6pm. This must be done for all potentially hazardous foods(eggs,chicken,cooked rice...) that have temperatures of 42-134.

No thermometers seen in 2 reach-in freezers.Please supply within week.Stem thermometer displayed 80 in ice water .Recalibrated.

Hot water at handwash sink which is to the left of the 3 bowl sink, can not be turned on at the handle but only at the supply valve below the handwash sink.
