Coleslaw was made to day approx 60lbs at 11am and product was not cooled to 41* within the 4 hours as required. Product internal temp was 49.4 and the ambient temp of the walk in cooler was 52*. Product was discarded.

Observed the ambient temp of the walk in cooler was 52* and the internal temp of coleslaw was 49.4* Called corp HQ for verification on slaw mix to see if it is a phf. There is approx 60lbs of coleslaw that is to be discarded.

Observed the counter laminate along the edge is in disrepair leaving raw exposed particle board.

Condensates from cooling unit of walk in freezer were leaking onto packaged product stored under unit. Product moved during inspection. Facility to correct within 7 days. No product storage under unit until corrected

Vacuum packaged seafood was not held in frozen state while in vacuum package. Facility advised that product is to be removed from package prior to thawing. Sale will be discontinued until safety barriers are verified.
