    Internal product temperature of items in bottom of pizza prep unit were not 41 or below as required. Cherries were at 46, pickles at 46 and pasta at 48 degrees F. Items in top of unit are below 41 - Cheese 37, ham 37. PIC said she would have maintenance look at unit.
  • Cleaning Frequency, Cleaning Agents & Clean Solutions for Mechanical or Manual Warewashing Equipment (46.674(a)-(c))
    Mechanical warewashing equipment had a residue buildup on the interior of the dish machine.
  • Food Temperature Measuring Devices Calibrated (46.691(b))
    Stem type thermometer indicated 27 degrees F rather than 32 when calibrated using ice point calibration. Adjusted during inspection.

    Drain from walk in cooler was separated and draining onto floor in front beverage area. Drains need to be going into drains and drains kept clean. Maintenance person re-connected drain.

    Small flying insects too numerous to count were in front waitress area around drink station and in back area around dishwasher. Insects need to be eliminated.

    Pizza prep topping table had condensation pooling in bottom of unit. It appears as if drain may be disconnected. PIC said he would have maintenance look at unit before leaving.

    Dough cooler beside pizza prep topping unit is operating at 50 degrees F. Employee had recently placed pans of pizza in refrigerator that he moved to walk in cooler.
  • Disposal of Sewage and Non-sewage Liquid Waste (46.863)
    Dishwasher drain is overflowing onto floor. Maintenance person was at facility and said he would check unit before leaving.
  • Insect Control Device Design & Installation (46.922(c))
    Several live ants and several dead ants were observed on counter where delivery computers were. According to PIC Larry Rice Ehrlich pest control (Randy Eckman) was at facility on Friday and treated for pests, however a pest control report was not available. Ants need to be eliminated from food facility.

    Moist wiping cloths observed lying on counter and not stored in sanitizing solution. Sanitizer solution was available on the counter.
  • Food Storage Requirements & Prohibited Areas (46.321)
    Condensation is dripping from the overhead refrigeration unit in large walk in cooler. Condensation is dripping onto racks below where packaged cheese was stored and onto floor by cart of pizza's. Cart was moved.
  • Frequency of Cleaning Nonfood-Contact Surfaces (46.714)
    Dough roller has oil leaking from out of the motor housing unit.
  • Availability, Capacity & Pressure of Water (46.805)
    There was no hot water available at facility at time of inspection. Hot water 77 degrees F. Dishwasher water at 77 degrees F. PIC stated that the hot water heater quit yesterday and that they had been operating without hot water.
  • Controlling Pests, Removing Dead or Trapped Birds, Insects, Rodents and Pests (46.981(k)(l))
    Ants too numerous to count were observed on the floor under the dishwasher.

    Most dishwasher racks have been cleaned however some still have a heavy build up of food residue on dishwasher racks. Dough roller has oil leaking from out of the motor housing unit.
  • Repair & Cleaning Various Premises, Structures, Attachments & Fixtures (46.981(a)-(c)(g)(n))
    There are some base tiles that are broken and missing from the wall behind the dishwasher and need repaired or replaced as there is water laying in the area around the wall behind the dishwasher. Soft Drink machine and ice bin drain is leaking onto floor and water is building up in areas between tiles where grout is missing.

    There is condensation dripping from the overhead refrigeration unit in the large walk in cooler. Condenasation is dripping onto racks below and onto the floor. Product needs moved away from this area as jugs of sauce are being stored under unit. PIC said that a part needed ordered.
  • Frequency of Cleaning Equipment Food Contact Surfaces and Utensils (46.712)
    Dishwahser unable to be checked as there was no hot water. Sanitizer concentration of mechanical dishwasher was at 0 ppm rather than 50 ppm as required for chlorine. Basin was set up to manually sanitize dishes that must be sanitized with chlorine at 50 ppm.
  • Frequency of Cleaning Nonfood-Contact Surfaces (46.714)
    There is a heavy build up of food residue on dishwasher racks. Racks need cleaned or replaced. Dough roller has oil leaking from out of the motor housing unit. There is still a dust build up behind walk in cooler refrigeration unit. Racks in walk in cooler have a dust and black milk like residue on them.
  • Availability, Capacity & Pressure of Water (46.805)
    There was absolutely not hot water at facility and no hot water tank in facility. PIC said that it had been removed earlier today as it was not functioning.
  • Repair & Cleaning Various Premises, Structures, Attachments & Fixtures (46.981(a)-(c)(g)(n))
    There are some base tiles that are broken and missing from the wall behind the dishwasher and need repaired or replaced as there is water laying in the area around the wall behind the dishwasher. Ice machine drain is leaking onto floor and water is laying in recess areas between wall and tiles where grout is missing.

PIC moved sauce and said they would sanitize bag before opening. There is a white mold like substance on area behind overhead refrigeration unit in large walk in cooler.
