Observed the hot water heater broke last night so there is no hot water available throughout the store and in prep depts. The store already had the depts shut down and no service was being conducted. A temp hand washing station using a cooler and heating water on the stove was set up in the bakery just to allow them to package product that was prepared yesterday.

Mechanical warewashing equipment observed not being operated according to the manufacturers data plate. The final rinse temp is not woirking. The mgr set up the 3 bowl sink.

Observed the portable fans in the deli and bakery have a build up of dust.

The food facility does not have a handwash sink located in the meat room.

Plumbing system not maintained in good repair - observed the faucet in the bakery is leaking.

Observed there are gaps underneath of the bay door approx 1inch allowing the possible entrance of vermin.

Observed the walls in the meat room around the walk in freezer and 3 bowl sink have a build up of pink slimy residue.
