The Person in Charge failed to notify the Department of an imminent health hazard and cease operations. Owner (food safety person in change) did not notify Dept. of not hot water present throughout establishment for the past 3-4 days and owner continued to operate under these conditions.

Food Employee did not follow proper handwashing procedure. Soap and/or warm water not used. No hot water present throughout establishment for proper handwashing.

Opened commercially processed ready to eat food (luncheon meats and cheeses), located in the refrigeration unit, and held more than 48 hours, was not marked. with the date it was opened.

All food equipment throughout the entire facility needs cleaning including cook, prep, refrigeration, storage equipment immediately. All equipment is has grease, food, and other debris build up.

Stagnant water inside containers not properly dispose contributing to fly infestation near dumpster area outside establishment.

Large amounts of refuse was observed in rear on ground surrounding dumpster, Citation # 1015365-1 was issued to owner.

Floors, walls, and ceiling all need cleaning. Build-up of food residue and grease are present.
- Toilet Room Cleaning, and Doors Closed (46.981(b)(i))
Entire employee restroom needs cleaning. Soiled toilet and sink fixtures was observed including walls, and floors are all in need of cleaning.
