PHF was held at 49 °F (sliced tom, Am cheese, deli meats) in the McCall prep cooler rather than 41°F or below as required. Deli meats discarded--remaining moved to other coolers. See item #31.
- Cooling, Heating, & Holding Capacities (46.611)
The McCall prep cooler is not maintaining PHF/ambient air at 41*F or less. Do not use for any PHF until repaired.
- Controlling Pests, Removing Dead or Trapped Birds, Insects, Rodents and Pests (46.981(k)(l))
Observed ground tunnels & five dead rats outside back door in the dumpster area. It was stated that the building owner may have placed bait out--empty black plastic trays were present. Facility does not have a pest control program. A professional pest company must be consulted immediately. The area in question & pest documentation will be reviewed upon reinspection. It is to be noted that no activity was observed on the inside of the restaurant itself.

- Chemical Sanitization for Mechanical or Manual Warewashing Equipment (46.674(d))
Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Discontinue use of unit. Switch to three bowl operation. All dishware in facility rewashed manually during inspection.
- Maintenance & Operation of Mechanical Warewashing Equipment (46.675(a)-(c))
Data plate on mechanical dishwasher indicates machine is a hot water sanitizing unit. Machine has been modified to operate as a low temp, chemical sanitizing unit. Facility to obtain letter from company who performed modifications that unit was modified according to NFS / ANSI standards. Replace data plate to indicate actual operating parameters. Observation was noted on previous report. Use of unit to be discontinued until verification that modification were made according to code are obtained and data plate is corrected or unit is converted back to a heat sanitizer. Contact Region 4 Office at 724-443-1585 when corrected.
- Light Bulb Protective Shielding (46.922(a))
Two - four foot fluorescent lights in front prep area are missing protective shields. Also noted one eight foot in same area was missing an end cap from protective shield. Correct within 5 days
