(Our main office is in Mesa AZ) Established in 1992 Hospice Family Care also serves the Phoenix metro area, including the West and East Valleys; Prescott/Cottonwood; and Tucson. Hospice Family Care provides physician nursing services, home health aids, social workers, pastoral counseling and counseling services to terminally ill patients and their families to provide support in this difficult stage of life. Our patients are able to receive help day or night through our telephone triage line and nurse visits are available when needed around the clock. Our goal is to take the worries out of caring for loved ones and to promote quality of life and dignity for all patients. Hospice care can be provided in the patient's home, assisted living, group home or nursing home. Our goal is to assist patients in maintaining their independence and to ensure their safety and comfort. The Medicare Hospice benefit pays for 100% of hospice under Part A.
