Permit to be issued via phone call to office.Permit will expire 11/2016.Restroom, Ceiling fan motor, not working correctly.(Monitor/Correct to proper operation). Ceiling tiles have evidence of roof leakage.

    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a QAC test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Equipment
    Observation: Stove/Grill burners were observed in a manner that contaminated the food product and fire hazard.( Heavy Grease buildup).
    Correction: Disassemble the Burners,clean and sanitize the food-contact surface, then reassemble the burners/area in a manner that will not contaminate the food and not be a fire hazard.

    Correction: Repair/Defrost/Clean the freezer to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the freezer, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: Encrusted grease deposits and/or other soil deposits were observed on the food contact surfaces of the following cooking equipment: (Deep Fryer Baskets).
    Correction: Clean and sanitize food-contact surfaces of deep fryer baskets, equipment and pans. Discard carbonized pans due to lack of cleanability.May procure/purchase new baskets if desired.
