Live cockroaches observed coming out of holes in equipment, from beneath ridges of three compartment sink, and from behind and in between pipes attached to walls. Various sized holes in walls present harborage sites. Methods are not being used to control pests. A certified pests controller has not been hired to address a potential cock roach infestation.

Comments:
All violations shall be corrected within 30 days. All food handlers must have a food handlers card from the health department. Food permit issued today.

Violation: 3220 - Corrected During Inspection Mops not hung up to air dry.
Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.
Comments:
All violations were corrected during the inspection.

Found the 2-compartment sink designated as the mop sink. If this is the case then no prepping of food or the handling of pots and pans can be done in the 2-compartment sink. Food Permit issued today.

Food Permit will not be issued until the rodent promlem has been addressed and the premises is cleaned up of its rodent remains. Also, plumbing problems must be corrected.
All critical violations are to be corrected within 10 days and non-critical violations within 30 days. A re-inspection to be made in 10 days.

Pest Control measures are being taken. Continue exterminating regularly even after infestation has been taken care of. Please continue to correct biolations as agreed upon. Dish sanitation should occur after dishes are cleaned and rinsed. PIC showed understanding of this process and agreed to use this procedure. 2006 permit issued.
