Permit displayed. Certified food manager (CFM) present. Previous violations corrected. Active managerial control observed. Facility organized and clean. HACCP plan observed and in compliance. Spoke with CFM regarding pest control, cleaning and maintenance schedules, food safety and general sanitation practices.
  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: The floor drain in the rear food prep room was observed in a state of disrepair.
    Correction: Replace the floor drain cover to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the floor drain, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Floor and Wall Junctures, Coved, and Enclosed or Sealed
    Observation: Floor wall juncture in dry food storage rom and throughout the kitchen is not coved and closed to no larger than 1/32 inch space.
    Correction: Cove floor wall juncture to no larger than a 1/32 inch space.
  • Pests - Controlling Pests*
    Observation: Harborage conditions exist. Approximately 65 live fruit flies observed resting on the walls, and flying throughout the facility.
    Correction: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. Clear floor drains of old, odorous decaying food in food prep room.

    Observation: Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: Rusty racks in victory cooler and debris on tray holder.

    Observation: Multiple missing wall tiles throughout the facility.
    Correction: Repair or replace wall or wall covering to make it smooth and easily cleanable.

    Observation: Multiple missing tiles throughout the facility, mold growing by hose in dishwasher room, rust on some walls next to floor and walls in walk-in cooler broken..
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Some intake and exhaust vents with accumulation of dust/debris.

    Observation: Observed accumulations of food deposits or other soil on the following: underside of solid metal shelving, underside of some tables, slicer.
    Correction: Clean and sanitize these surfaces.

    Observation: Physical structure is not maintained in good repair: broken baseboards, loose wall panels in walki in #6.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. *Work requests have been submitted.

    Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health as they relate to the symptoms caused by illness, infection, or other source that is associated with an acute gastrointestinal illness or a lesion.
    Correction: Have the license holder require employees or applicants report to the person in charge if they are experiencing the following symptoms or conditions: (1) diarrhea, fever, vomiting, jaundice, or sore throat with fever. (2) A lesion containing pus such as a boil or infected wound that is open or draining and is on the hands or wrists, unless an impermeable cover such as a finger cot or stall protects the lesion and a single-use glove is worn over by an impermeable cover or on other parts of the body, unless the lesions covered by a dry durable, tight-fitting bandage.

    Observation: The nonfood contact surface of the crates used for shleving/storage is not designed or constructed to be easily cleanable.
    Correction: Alter or replace this item to provide a surface that is free of unnecessary ledges, projections, and crevices.

    Observation: The door gasket to the Victory Refrigerator is in poor repair.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.

    Observation: Faucet in poor repair.
    Correction: Repair and maintain all plumbing components ans fixtures.
  • Sewage - Other Liquid Wastes and Rainwater
    Observation: A condensate drain line was not draining properly causing ice build up in the freezer.
    Correction: Repair the condensate drain.

    Observation: A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees

No violation noted during this evaluation.
03/06/2013Risk Factor
Dishmachine was under routine maintenance during inspection. Record shown that rinse temperature was measured at up to 200 F on the thermometer. Person in charge is advised to adjust boost heater temperature to maintain final rinse temperature between 180 - 194F to provide proper sanitization of the utensils. Overall conditional of the facility is sanitary during inspection.
  • Toilet Rooms - Enclosed
    Observation: Toilet room door is not tight fitting. and/or provided with a self- closing door
    Correction: Provide toilet room door that is tight fitting. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas.
