Supplier is US Food. Discussed employee health. Form 1B posted and employees have reviewed and signed. Director stated form is reviewed every several months. No raw proteins prepared. No foods are cooled and held over for next day service. No raw eggs prepared. Juice is pasteurized. Milk is shelf stable until opened. Canned and frozen vegetables used. Discussed handwashing and glove use. Discussed washing fruits and vegetables. Discussed major risk factor that would be of concern in facility. Discussed upcoming changes in the regulations including food employee training. Food is portioned by PIC in kitchen and dispensed by teachers in the classroom. Teachers are trained to wash hands and use gloves when serving food. Children wash hands prior to eating.

Discussed Employee Health and observed posted From 1B and signed Form 1B for each employee in facility. Facility does not hold over leftovers. Terminex is LPCO. Discussed cleaning and sanitizing food contact surfaces & utensils. Recommend wearing hair restraint during any food preparation.

Follow up inspection for pest issue. No evidence of pests (mice) observed. PIC has provided records from the LPCO (licensed pest control operator). Per PIC, LPCO is coming to facility weekly. Recommended the following: continue to cover all dishes, equipment and utensils at night

    Observation: Observed dishes and utensils not being sanitized before use and after cleaning. Observed the chlorine sanitizer in the 3 basin sink at a concentration too low
    Correction: observed at or less than 10 ppm.
  • Pests - Controlling Pests*
    Observation: Observed premises not being routinely inspected for evidence of pests. Observed mouse droppings on fruit juice container.
    Correction: Recommend inspecting premises on routine basis for the evidence of pests. . Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. PIC stated that licensed pest control operator (LPCO) had been contacted and is working to correct the issue. Discussed checking food containers and general facility to ensure that there is no evidence of contamination by pests. Discussed covering dishes and equipment. PIC discarded fruit juice containers. PIC asked to submit pest control records to EHS.
