July 02, 2007 (Complaint)

Comments:
This visit is the result of a complaint regarding a dirty dining room (carpet), live roaches, food left out over night. Carpet is very old and stained in the dining room. Will need replacement when possible. Food (soup) left out on table from last night (cleaned). Food must be cleared from tables every night to avoid harborage of pests. Observed dead roaches on floor in dining area (no live activity). No pest control operator used-need to contract professional pest control for monthly treatment.

The following have been corrected: Soap has been supplied inthe female restroom; date markings have been supplied for potentially hazardous foods held for at least 24 hours; thermometers have been supplied in the reach-in refrigeration units; the front door is closed; wiping cloths are clean; single serve items in the dining room are at least 6" off of the flooring; the badly torn pan on the cooks line has been discarded; the dirty rolling dish cart on the cooks line has been cleaned; some of the walls alongside of and around the cooks line have been cleaned; most of the unnecessary equipment not being used in the dining room has been removed.
