When they are experiencing any of the following symptoms: vomiting, diarrhea, sore throat with fever, jaundice, and any lesions containing pus such as a boil or infected wound that is open or draining and is on the exposed portions of the arms, hands, or wrists; 2). Diagnosed illnesses caused by: Salmonella, Shigella spp., E.coli 0157:H7, Hepatitis A Virus, or Norovirus; and, 3). Potential exposure to: Salmonella, Shigella, E.coli 0157:H7, Hepatitis A virus, or Norovirus. Based on the information given, the person in charge shall then restrict or exclude the employee from working as a food employee.     Mark the name and ""consume by"" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41F or below the food shall be served or sold within 7 calendar days or within 4 calendar days if held at 45F.   • 3360 - Corrected During Inspection Critical Flying insect spray improperly stored in foodservice area.
    Discarded. Advised only approved insecticides allowed and must be applied by Certified Pest Control Operator and stored separate from general cleaners.
Comments:
Shelving removed from beer room and storeroom and replaced with coated wire shelving or metal shelving. 2006 permit issued; post in public view and review emergency instructions on reverse side with all staff.

    Operator has 60 days from the date the interim permit is issued to remove wood shelving and replace with metal shelves.
  • 1570 - Wood drawers no longer sealed.
    Seal the drawers or remove the unit.

    Completely remove the unit or replace it.

  • 2350 ii - One of the waste drain lines of the three-compartment sink is leaking.
    Repair the drain line.

    All refuse containers are to be stored on smooth, non-absorbent material that is sloped to drain. Remove overgrown vegetation.

    All refuse containers are to be stored on smooth, nonabsorbent material that is sloped to drain. Operator has 60 days from the date the temporary permit is issued to provide proper pad.
  • 2660 - The women's restroom does not have a covered trash receptacle.
    Provide a covered trash receptacle in the women's restroom.
  • 2790 - The concrete floor in the storeroom is not sealed.
    Seal the floor or install approved floor covering.
  • 2830 - The floor-wall juncture along the wall/behind the combination sink is not sealed.
    Seal the floor-wall juncture or install cove molding.
  • 2920 - The women's toilet room door is not provided with a self-closing door and is propped open.
    Provide a self-closer for the toilet room door and adjust it so the door closes tightly. Do not prop toilet room doors open. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas.
  • 2930 - The back door is self-closing and there are gaps around the door frame.
    Repair/replace the self-closing device and eliminate the gaps around the door frame.
  • 3080 - Lighting is insufficient in the foodservice area and storeroom.
    Operator has 60 days from date of temporary permit issuance to upgrade lighting to 50 food candles in the foodservice area and 10 foot candles in the storeroom.
  • 3080 - Lighting is in poor repair above back hallway.
    Restore lighting to operation.
  • 3080 - Lighting not provided:-below vent hood-inside homestyle freezer
    Operator has 60 days to install sufficient lighting below the vent hood system. When the freezer is replaced, it must be replaced with one that has sufficient lighting installed.
  • 3170 - The following physical structures are in poor repair:-outlet cover is missing to right of barside low boy -threshold missing at back door-floor below equipment is not completely tiled-hole in wall below coffee brewer-wall not secured behind combination handsink -gaps exist around plumbing pipes below the three-compartment sink and women's restroom -light fixture loose on hallway ceiling-two holes in ceiling above back door
    Maintain physical structures in good repair. All items to be repaired within 30 days of the issuance of the temporary permit.
  • 3220 - Mop rack not provided.
    Provide a utility rack to hang mops after use.
  • 3300 - Facility is storing unnecessary items to the operation or maintenance of the establishment.
    Remove all unnecessary items from the facility. The presence of unnecessary articles, including equipment which is no longer used, makes regular and effective cleaning more difficult and less likely. It can also provide harborage for insects and rodents.
Comments:
Original evaluation conducted 4/26/06 as routine evluation under previous owner and ownership change. A temporary permit is issued and will expire in 30 days; operator has 30 days, except where otherwise noted, to make the necessary corrections and repairs. A follow-up evaluation will be conducted at that time to ensure correction of 30 day repairs.
