Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.

Repair or replace floor or floor covering to make it smooth and easily cleanable.
3380 - Corrected During Inspection Critical Quaternary ammonium being applied to food contact surfaces does not meet the requirements of 21 CFR 178.1010. The concentration was 0 ppm.
Utilize only quaternary ammonium that meet the requirements of 21 CFR 178.1010 when applying to food contact surfaces. The concentration needs to be 200ppm.

Comments:
Educated staff in backflow prevention in regards to the 3-vat sink and the spray nozzle.
Observed good used of gloves.
Suggested food school at Lynchburg Health Department.
