Violation: 2890 - Light bulbs over fry station not shielded, coated, or otherwise shatter-resistant. Shield or replace light bulb with a coated or shatter-resistant bulb.
Comments:
Paperwork Needed: Employee Health Policy *Food prep sink 4-VAT needs at least a one inch air gap at connection to sewer to prevent backflow, reference the Five Guys near Wal*Mart for proper setup.* -Be sure to keep mop hose above flood rim to prevent backflow. -Ensure only simple wedding bands are worn with gloves for food preparers. ~Facility was very clean and well kept at time of inspection. Good Job! Comments: Facility uses CFL (Compact Florescent Lights) ensure employee's are briefed on proper cleanup procedures. Observed glove use, observed food labeling. Quat appeared 250 PPM. Quat bucket in sanitizer is for veggie prep, chlorine sanitizer is for meat prep. Meat table (L), Veggie table (R), foods are always on pans. Facility does not do veggie/meat prep at same time, when has to tables are pulled apart. Hot holder (mushrooms/onions) is time controlled, discarded every four hours. Facility has acquired a digital thin diameter probe. Chlorine 100 PPM.

Violations: - 2350 ii - Plumbing connection at front handsink is leaking.
Plumbing systems and components shall be maintained in good repair.
- 3330 - Critical Working containers of cleaner are not properly labeled.
Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.
- 3360 - Corrected During Inspection Critical The cleaner is not being used in accordance with law or the manufacturer's use directions.
The cleaner must be used in accordance of law and in a manner that does prevent the contamination of food, equipment, utensils, linens or single service items.
Comments:
Observed use of gloves and hair restraints. Pest control program in place. Suggest purchasing a thin diameter probe thermometer, needed for hamburger temperatures. Discussed employee health policy-enforceable by April 1. Hamburgers are not served undercooked no need for consumer advisory. In Walk-In: patty's, tomatoes, and lemon prepped this morning. Suggest not stacking plastic containers by beef.
February 09, 2007 (Routine)
Comments:
*No Violations Observed At Time Of Inspection.* Quat-200 PPM Mops were inverted and good date marking. Good Job! Be sure to only use food grade gloves for food prep, not gloves marked for medical use. Be sure that employee lunches are kept away from food served to public. Be sure sanitizer buckets as well as all foods are stored off floor on approved shelving. Be sure hole in ceiling near fry washing area is fixed and that hose wrap at mop sink is installed. Don't forget to keep dumpster door closed. Meat prep table (perhaps sign alerting for meat prep only). *Previous trash problems have been resolved with installation of secondary dumpster.*

Violation: 2740 - Trash and litter were observed adjacent to the refuse container outside the facility. The refuse container storage area and grounds adjacent to the container are to be maintained clean and sanitary.
Comments:
Complaint- Dumpster is full and garbage out on ground. - Garbage observed sitting around dumpster as well as behind the facility - Trash pick-up is done 4 times a week - Need to either have pick-up done more frequently, another dumpster, or a dumpster for card board only - Complaint verified
January 11, 2007 (Pre-Opening)
Comments:
*Hood filters cleaned every night, entire system flushed every 3 months.* -Sevsafe information will be in store by next inspection. -Pest control receipts as well as water bill acquired for file, will be opening around January 12th. -Dumpster location observed - Dumpster empty at time of inspection, be sure to keep dumpster door closed during operation. - Construction dumpster will be gone tomorrow. - Grease disposal methods observed. -Soap/towel dispensers observed at all employee handsinks and customer restrooms. Ladies restroom has covered trashcan. Handwashing signs have been observed in both walk-in and prep table at warmest area close to door. -Adequate dry storage is in place, cleaning supplies are stored in separate rooms from food. -Rear exit door no longer has gap at bottom. -All handsinks/3-vat are sealed to wall, bottom of wall board has been sealed with grout to tile. -Sanitizer test strips, food grade gloves, and hair restraints observed. Comments: Prep table top door was observed not closing all the way, problem was corrected on-site by cutting board readjustment. Be sure to do final site cleaning before opening. Be sure to invert mops not in operation (mop hanger and hose hanger to be installed). Not using drive thru. Be sure all cleaning containers are labeled.

Comments:
*Please obtain a copy of CO when acquired.* Potential water leak has been reported to county. *Pest control receipts needed as well as water bill receipt needed for file.* Wanting to open at the end of next week. Food and stock will be here on Friday for the pre-opening. Backflow devices installed on main and faucets. -Same dumpster location as Shoney's-be sure is located completely on pavement. Grease disposal to be acquired. -Soap/towel dispensers to be installed at all employee handsinks as well as covered trash receptacles need to be placed in ladies restroom. Don't forget to place handwashing signs at each handsink including restrooms. -Thermometers need to be placed in each refrigeration unit (even if has external thermometer). Place these close to the door in order to monitor temperature in warmest part. -Please ensure all dry storage is up by pre-opening and that the shelving is easily cleanable and allows all foods to be stored at least 6 inches off of the ground. Cleaning supplies and food are stored in separate rooms. Employee items will be placed in supply room. -Rear exit doors need to not have a gap that would potentially allow for pest entry. Parts will be installed to ensure gap is covered. -Please ensure all handsinks and 3-vat are sealed to wall with caulk/sealant to ensure no grim/debris can be acquired between sink and wall. *Don't forget sanitizer test strips. *No dishwasher, will use 3-vat sink to wash/rinse/sanitize.
