This inspection was a risk factor inspection that focused on the 5 risk factors for foodborne illness (food from unapproved sources, inadequate cooking temperatures, inadequate holding temperatures, improper employee health and hygiene, and cross contamination of food and equipment). Good retail practices (GRPs) observed were discussed with operator and/or staff at time of evaluation including cleaning and maintenance of equipment and physical structures.     Observation: The interior of the ice machine was soiled. Gaskets on multiple units were soiled.
    Correction: Clean as needed to prevent accumulation of grime and debris.

    Correction: Educate school staff to close lids after throwing trash away.

    Observation: The door gasket to the the reach-in cooler is torn and soiled.
    The utensil drawer was rusted with utensils stored in it.

    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.
    Rusted utensil drawer is to not be used until a non-absorbant liner can be placed inside the drawer.
  • Non-Food Contact Surfaces (repeated violation)
    Observation: The following were noted in the need of cleaning:
    1. Fan above the back door had accumulations of dust.
    2. Door gaskets on reach-in cooler number 2.
    3. Ice machine

    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    1. Hand sink in employee rest-room.
    2. Hand sink in the kitchen.

    Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage.

    Observation: 1.Light cover located at the end of the serving line was observed in a state of disrepair and damaged.
    2. Inside of utensils draw corroded/rusty throughout.
    3. Reach-in cooler number two had a torn gasket.

    Correction: Repair.
  • Non-Food Contact Surfaces
    Observation: The following were noted in the need of cleaning:
    1. Fan above the back door had accumulations of dust.
    2. Door gaskets on reach-in cooler number 2.

    Correction: Clean.
  • Handwashing - Using a Handwashing Lavatory (corrected on site)
    Observation: The handwashing facility located in the kitchen was blocked by a trash can, preventing access by employees for easy handwashing.
    Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the trash can preventing its use.
  • Refuse - Covering Receptacles (corrected on site)
    Observation: Outside dumpster was open.
    Correction: Keep all waste closed when not in continuous use.

    Observation: The wooden shelf surface at the serving line has deteriorated.
    Correction: Replace. All equipment should be durable, smooth, easy to clean, and non absorbable.

    Observation: 1) A large plastic pitcher has a melted base.
    2) The left door gasket on the 3 door refrigerator is damaged.
    3) The surface of the chemical storage table at the 3 compartment sink is damaged.

    Correction: Replace pitcher( Corrected.)
    Replace door gasket.
    Replace table with equipment that is durable, smooth, easy to clean, and non absorbable.

    Observation: The dumpster lid was open during the inspection.
    Correction: Keep dumpster lids and doors closed when the unit is not being serviced.

    - gasket is torn on the three door refrigerator

    Observation: The pressure gauge for the high temperature warewash machine was above 25 PSI.
    Correction: Immediately discontinue the use of the machine until it is operating in accordance with minimum requirements. The range for proper operation is between 15 and 25 PSI. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils. To minimize workload you can move to all single service utensils.

    Observation: The floor under RF1 needs cleaning.

    Observation: The water temperature in the manifold servicing the high temperature ware wash machine was below the minimum allowable level of 180ºF. The thermometer read <165ºF>.
    Correction: Immediately discontinue the use of the machine until it is operating in accordance with minimum requirements. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils.
  • Light Bulbs Protective Shielding
    Observation: Light bulb in the 3 door freezer is not shielded, coated, or otherwise shatter-resistant.
    Correction: Shield or replace light bulb with a coated or shatter-resistant bulb.
  • Physical Facilities in Good Repair
    Observation: There are small holes in the wall behind the dish machine.
    Correction: Fill the holes so that the wall is smooth, durable, easily cleanable and nonabsorbent.
