Violation: 3270 - Critical Repeat Roaches were still observed in the back utility area and storage area, though they were fewer in number than on previous inspections.
Continue with pest control efforts until roaches are eliminated.
Comments:
This was a follow-up inspection to assess progress of pest control efforts to eliminate roaches as noted on previous inspections.

  • 3270 - Critical Repeat Though fewer in number, roaches were still present in the establishment.
    Continue with aggressive treatment and rigorous cleaning.

  • 1800 - Repeat Many pieces of equipment were not clean (grease and crumbs), esp. in the hard to reach areas.

  • 3270 - Critical Repeat Roaches were observed near the staircase and in the utility room. They did not seem as numerous as on previous inspection.
    Take all necessary action to eliminate roaches. Eliminate harborage areas, thoroughly clean floors and equipment, and continue with professional pesticide applications.
  • 3280 - Many dead roaches were observed on the floors.
    Remove dead roaches.
  • 3170 - Repeat There were many gaps along the staircase.
    Repair / seal all of the gaps.
  • 3170 - Repeat The walls were damaged in many areas (staircase, under chemical closet, behind carbonizer, etc.).
    Repair the walls.
  • 3170 - Repeat The grout was severely eroded in the mopsink.
    Repair / regrout the mopsink.
  • 3170 - Repeat The kitchen ceiling tiles are worn.
    Replace worn tiles.
  • 3180 - Repeat The floor was not clean in areas.
    Clean and maintain the floor.
Comments:
Although less in number, roaches were still observed. That and the other repeat violations require correction before a 2004 permit can be issued. The 2003 permit will expire December 31st.

  • 1800 - Repeat The fryer cabinets were not clean.
    Clean and maintain the fryer cabinets.
  • 1800 - Many pieces of equipment had grease / crumb build-up in hard to reach areas (gaps, ledges, legs, and wheels).

    Remove the ice build-up / defrost.

    Replace the ceiling tiles over time.
  • 3170 - Repeat The floor grout was eroded in the mop sink.
    Restore the eroded grout.
  • 0550 - Ice scoop handles were allowed to fall into ice at the drive through.
    Keep scoop handles up and out of food.
Comments:
The certified food managers certificate was not on display in view of the public. Post the certificate.

August 22, 2003 (Routine)



Violations:
  • 3270 - Critical Roaches in all stages of development (juvenile to adult) were observed in many areas of the restaurant.
    Immediate corrective action is required to eliminate the roaches from the establishment. This action must include thorough cleaning, removal of harborages, and professional extermination services.
  • 1770 - Repeat The interior of the ice machine was not clean behind the plastic guard.
    Clean and sanitize the interior of the ice machine. Removal of the guard is required.
  • 1770 - The water left in the unused holding unit on top of the freezer was dirty (dead roaches, food debris).
    Remove the water from the unused holding unit and clean as frequently as needed.
  • 1800 - The non-food contact areas of the freezer with holding unit on top were not clean (mechanical area on side, around fittings for shelves).

  • 1800 - The unused sink across from the 3-compartment sink was not clean.

  • 3180 - Water was pooling on the walk-in refrigerator floor.
    Remove the water from the walk-in floor.
  • 3180 - The ceiling was dusty in areas around air returns.
    Clean and maintain the ceiling around returns.
  • 3180 - The make up air vent was dusty.
    Clean and maintain the make up air vents.
  • 3300 - Unused ceiling tiles, etc. were being stored below the chemical storage closet.
    Remove unnecessary items from the restaurant.

May 05, 2003 (Routine)



Violations:
  • 0820 - Critical Gravy and chili in the hot holding unit were measured at 132*.
    All potentially hazardous foods must be kept at 140* or above. Corrected during the inspection.
  • 1770 - The roof of the ice machine was not clean behind the plasic guard.
    Clean the roof of the ice machine behind the plastic guard.
  • 1770 - The utensil storage drawer had water pooling in the bottom.
    Remove the water and clean the storage drawer. Corrected during the inspection.
  • 2000 - A box of straws was observed on the floor under the storage racks in the upstairs storage area.
