Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

Instruct food employees to clean their hands in a handwashing lavatory. Food employees may not clean their hands in a sink used for food preparation or utensil washing.
0930 - Critical A review of the menu with the foodservice operator indicates that there is no consumer advisory for the steaks that may be served raw and/or undercooked If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means.
1530 - There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions. Obtain a chlorine test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
1570 - Kitchen handsink soap dispenser not in good repair. Repair the soap dispenser to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the soap dispenser, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

2350 ii - The faucet of the 3 compartment sink is not long enough to reach all three sink basin Replace 3 compartment sink faucet to ensure the proper use of the 3 comp sink for washing, rinsing and sanitizing food contact surfaces of equipment and utensils.
3170 - The kitchen floor (by the grill) is not maintained in good repair Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
3260 - Observed employee personal items (hats) stored on the clean equipment shelves. Employees that regularly change their clothes in the establishment should use the dressing room or lockers provided.
Comments:
Water capacity restored. Employee health policy in place. The major changes of the food regulations, risk factors for foodborne illness, including employee health and consumer advisory also proper handwashing were discussed with manager. Handouts with information was provided.

Violation: 2120 - Critical The water source and system serving this food service establishment has been interrupted. EHS is to confer with his/her supervisor for closure of establishment for an imminent health hazard. (See section 12 VAC 5-421-3910)
Comments:
Permit suspended until water capacity be restored. It was requested that PIC contact the health department for opening/ follow up inspection.

Violations:
0570 - Repeat Wiping cloths improperly stored between use. Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.
0580 - Gloves not being worn for food preparation Gloves must be worn when handling food.
1570 - Handsink was observed in a state of disrepair and damaged. Repair the hot water valveto restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the valve, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
2190 - Water from the handwashing sink at prep area was measured at a temperature less than 110F. Make necessary adjustments to valves and lines serving the handsink to provide water of at least 110F to allow more effective removal of fatty soils encountered in kitchens.
3330 - Critical Working containers of spray bottles are not properly labeled. Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.
Comments:
Handsink must be repaired ASAP to allow for hot water when washing hands. Spoke with chef concerning glove use and how gloves must be worn when preparing food.
