January 28, 2009 (Routine)
Violations: - 3220 - Mops not hung up to air dry.
Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.
- 3660 - Permit is not posted in a place where it is readily observable by the public.
Post the permit in a place where it is readily observable by the public transacting business with the establishment.
Comments:
Check mini refrigeration in drive thru, thermometer my need replacing. Besure to invert or cover all single serve items to prevent cross contamination. Check food contact equipment and utensils, observed several plastic containers broke or cracked, replace. Per 12 VAC 5-421-570; wet wiping cloths when not in use need to be stored in a sanitizing solution at the required concentration. Make sure to clean and sanitize all food contact surfaces not temperature controlled every four hours or as needed to prevent debris build-up. All other cleaning and sanitizing needs to be done every 24 hours or as needed to prevent debris build-up.

Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.

Access to the handwashing facility identified above is to be available during all hours of operation. Remove the receptacle preventing its use.
Comments:
Intake fan in the Walk-in needs to be cleaned. Make sure all food when stored, is 6" off the ground, items stored on the floor in the walk-in freezer. When stored all food needs to be covered, biscuits uncovered in walk-in freezer. Sanitation buckets though 6" off the floor need to have easy access underneath for cleaning.
January 10, 2008 (Follow-up)
Comments:
Received food safety sheets from 1/9/08, all temperatures are not in the danger area between 41 and 135. Need to clean and sanitize all equipment and food contact surfaces around repair area. Check the entire area around the repair for both contamination and potential contamination. Filters on the hood system need to be cleaned to ensure chemical as well as dry wall are not in the filter and can fall from above. Wall at the spot of the repair needs to be smooth and easily cleanable per 12 VAC 5-421-2810.
January 09, 2008 (Critical Procedures)
Comments:
Sheriffs department called in to inform Health Department of a minor fire at the KFC/Taco Bill #19680. This electrical fire occurred behind a "holding cabinet" in between the drive-thru handsink and fryer units. Fire was extinguished with dry chem extinguisher. County Fire Marshall was notified and power to the facility was shut down temporarily to isolate problem. Power never actually went off to middle area where most of the refrigeration units are. Power was off to walk-ins around 10-20 minutes and doors were kept shut per general contractor. Facility has been given permission from Fire Marshall to reopen pending Health Department approval. All equipment will continue to function as normal operation except for "holding cabinet" which will remain off-line pending building inspection. Outlet will also remain off-line by fryers, wall will be repaired tomorrow. Facility has cleaned area where extinguisher and has discarded all foods that were out when extinguisher was discharged and/or has been out of temperature longer than four hours. Any single service items that could have been contaminated by extinguisher have been discarded. Facility even replaced fryer oil which could have been contaminated. Facility plans on re-opening later this evening. Facility did temperatures at walk-in when power was restored and it was 41 degrees or below but did not record them. Complete food safety audit was to be conducted prior to opening and will be available for file. Any foods that were salvaged were on the opposite side of kitchen from discharge area and were sealed. All repairs were to be completed by 01/10/08 and will be verified by follow-up inspections.
August 13, 2007 (Critical Procedures)
Comments:
Complaint: Ate at Taco Bell on Thursday 8/9/07 around 1:00 pm. Became ill the next day and friend also got sick. Inspection interview with management: - No other complaints received. - No bad shipments received. All shipments are inspected by staff upon arrival, no drop key deliveries. - Any shipments that are bad or up to company standards are sent back to distributor. Manager has to sign for all shipments sent to facility. - No Environmental issues regarding water and air. - If ill, employees are asked to call out. A Dr. Note is needed if employees are ill for two or more days. - Process for making foods have not changed. No new equipment have been used. - Shipment received every two days. - All food prepared at distributor and reheated at facility. - People not cross trained people who handle raw chicken just handle raw chicken and nothing else except cleaning. Inspection notes: All temperatures were taken by management with EHS supervision. Drive thru dispenser cleaned by Pepsi Co. All chemicals are stored in back by hot water heater. Gloves are mandatory via company policy. Dispenser are cleaned nightly by staff. Upon inspection no issues were found, therefore complaint could not be verified.
April 16, 2007 (Routine)
Violations: - 0610 - Food stored on the floor or food stored less than 6" above the floor in walk-in freezer.
Elevate food storage onto approved shelving with minimum 6" legs or casters.
- 2000 - Single service items observed unprotected from contamination.
Store single service items in its original protective packaging or inverted in an approved dispenser.
- 3220 - Mops not hung up to air dry.

Comments:
Temperatures continued: Henny Penny hot hold by 3-vat sink 150 Facility clean during inspection. Also recommend that water heater be serviced, water is leaking from copper pipe. Recommend having thermometer in easily accessible areas in all refrigeration units.

Violation: 2890 - Light bulb over the service area counter are not shielded, coated, or otherwise shatter-resistent. Shield or replace light bulb with a coated or shatter-resistant bulb.
Comments:
Be sure to check all canned goods for dents. Facility clean upon inspection.

Comments:
Remodel inspection/Construction inspection - Back splash guard needed on back handsink by walk-ins. - Make sure back flow on all equipment that is hooked up to water supply that will be served to the public. - All finals need to be complete before pre-inspection. - Temporary stocking permit needed to bring food into building, obtain from building. - All cold hold units must be turned on and holding at 41 or lower. - Once Health Department permit issued VDH will need a copy of Cert. of Occupancy under regulation 12 VAC 5-421-3660A.
