The purpose of today's visit is to conduct routine inspection.
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.--CFM arrived during the inspection.

Sanitizer: for 3-Vat and wiping towel buckets chlorine (Bleach) > 50 ppm.
Pureforce PA-1 Dish washing Machine: Chlorine > 50 ppm.
Provided EHP document package in English and Spanish.

  • Critical: Consumer Advisory Required for Raw or Undercooked Animal Food (corrected on site)
    Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/or undercooked: WIndow and carry out menus were correct. Table menus (newly printed) had asterisks but no test euc to printer error.
    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. PIC printed labels during inspection and corrected table menus.

The hood exhaust system is due for cleaning within two weeks. The owner stated that cleaning was scheduled for the night of December 4, 2013. Fax or email a copy of the work order to the Health Department within 72 hours.
**Approval is hereby granted for the issuance of a Health Department Permit. This inspection report shall serve as your permit until the official permit is received. All outstanding violations are to be corrected within ten (10) days but no later than the time of your first routine inspection (approximately 30 days). Equipment additions/replacement/changes are not allowed without Health Department approval.**
Note the following recommendations:
It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.
Recommendation is made to have exhaust hood cleaned at least every six months. Maintain records of hood cleaning for Health Department review.
Recommendation is made to clean hood filters at least every two weeks.
It is recommended that the grease trap be cleaned at least every two months. Maintain records of grease trap cleaning for Health Department review.
It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.
Water heater: Rheem Ruud G75-125

  • Equipment / Good Repair / Components / Gaskets (repeated violation)
    Observation: The walkin refrigerator door does not close on its own. The closer mechanism is missing from above the door.
    Correction: Install a closer on the walkin refrigerator door. Part has been ordered - receipt provided during inspection.
  • Hand Drying Provision / Individual, Disposable Towels (repeated violation)
    Observation: There are not paper towels available at the handsink at the bar.
    Correction: Install a paper towel dispenser at the bar handsink.
12/04/2013Pre-Opening
Remove the paper towel dispenser from above the 3 vat sink at the bar. Handwashing must only be done at the handsink.
Approval for a Health Department Permit to Operate is denied at this time. Review the cited items above. All items must be corrected at the time of the next pre-opening inspection. Contact the Health Department in order to schedule a re-inspection.
Per the owner, a new dishwasher is to be installed. Cut sheet was provided and unit was approved for installation.

    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Fax or email a copy of the CFM card to the Health Department within 10(ten) days.
  • Critical: Direct Connection Not Allowed / Backflow Prevention Required/ 3-Vat Sink & Food Equipment
    Observation: There is a direct connection from the sanitize sink to the grease trap.
    Correction: Install an air gap on the drain line from the sanitize sink to the grease trap. The design of the rinse sink is exactly what is needed at the sanitize sink.
  • Floor , Wall & Ceiling / Design
    Observation: The ceiling tile located in @LOCATION@ is absorbent and not easily cleaned.
    Correction: Wall, floor, and ceiling surfaces shall be smooth, durable, easily cleanable, and non-absorbent for areas subject to moisture such as food preparation areas, walk-in refrigerators, warewashing areas, and toilet rooms.
  • Floor / Wall / Ceiling / Cleanability
    Observation: All ceiling tiles, and some vertical surfaces, in the food prep area are not smooth and easily cleanable.
    Correction: Replace all ceiling tiles, horizontal and vertical, in the kitchen.
  • Toilet Rooms, Enclosed / Self-Closing Door
    Observation: There is not a closer on the men's restroom door.
    Correction: Restroom doors must be self-closing. Install a closer on the restroom door.
  • Hand Drying Provision / Individual, Disposable Towels
    Observation: There are not paper towels at all handsinks.
    Correction: Provide paper towels at all handsinks.
  • Physical Facilities Good Repair
    Observation: There is a buildup of mildew along the back of the 3 vat sink and along the back of the 2 vat prep sink.
    Correction: Remove the old caulk and re-caulk along the back of the 2 vat and 3 vat sinks. Remove "valleys" which might collect water and lead to a mildew problem again.
  • Ventilation Systems, Shall Be Clean
    Observation: The hood is due for cleaning within two weeks.
    Correction: For the purpose of this inspection, fax or email a copy of the invoice for the hood cleaning to the Health Department within 14 days.
