    Observation: Soda lines in poor repair. Lines were observed leaking in basement area. Coke is aware of problem and parts are on order.

    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Observed methods are not being used to control pests. Flying insects observed in basement storage area.

    Observation: Improper handwashing procedures observed. employee observed washing hands without soap.
    Correction: Ensure all food employees clean their hands and exposed portions of their arms with a cleaning compound in a lavatory by vigorously rubbing together the surfaces of their lathered hands and warms for at least 20 seconds and thoroughly rinsing with clean water.
  • Plumbing System Maintained in Good Repair
    Observation: Beverage lines in poor repair. Liquid was observed leaking along beverage lines from above. Bucket was observed placed on shelf to collect flow. Mold/mildew/slime was observed accumulationg on outside surface of lines.
    Correction: Repair and maintain all plumbing components ans fixtures.
  • Outer Openings - Protected
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents. Drive thru window is broken and no air curatin is present. Per manager replacement is on order.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Handwashing Cleanser - Availability (corrected on site)
    Observation: Soap was not provided at the hand washing lavatory in the kitchen
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.

    Observation: No disposable towels were provided at the hand washing lavatory in the McCafe handwash sink.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. Items were moved to another cold holding unit and cooler was removed from line.

1 - Arrival on site for a routine inspection revealed: 1.) approximately 2 inches of sewage on the floor of the basement, 2.) drain near bag in box system appears to be completely backed up, 3.) sewage pipes between first floor and basement are leaking, 4.) sump pump appears operational, 5.) all paper goods, dry goods and the walk-in freezer are located in the basement.
2. - Consultation with manager on duty (Angela) revealed: 1.) problem has been occurring since the previous night, 2.) employees are manually pushing sewage around the floor, 3.) plumber called but expected arrival time was unknown.
3. - Immediate recommendations to manager on duty: 1.) segregation of basement area to prohibit employee traffic, 2.) food service to continue only with supplies available on the first floor, 3.) contact owner for corrective actions and contact plumber for estimated time of arrival, 4.) advised manager of responsibility to contact health dept. with imminent health hazards.
4. - Consultation with owner Kyle Harmon. Discussed imminent health hazard. owner agreed to limitation on food service through lunch hour. Discussed recommendations for voluntary closure. Owner elected to continue service through lunch or until supplies are exhausted. Owner understands that access to basement is prohibited.
5.- Consultation with plumber. plumber agreed to assess the situation and advise health department of corrective actions and tentative time for completion/resolution of sewage problems.
6.- Plumbers unclogged the drain in the basement and corrected the plumbing issues with the drive thru drink station and the McCafe machine which were the source of the leaking pipes. McDonald's employee cleaned up water and cleaned all affected areas with soap and water followed by a bleach water sanitizing step.
7. - Basement was reopened to employee traffic.

  • Critical: Sewage - Conveying Sewage* (corrected on site)
    Observation: Sewage is not being properly disposed of in an approved sewage disposal system.
    Correction: Immediately discontinue operation until sewage waste can be properly disposed of. Approval from the health department must be received prior to resuming operations. The permit holder is responsible for eliminating any public health hazard or nuisance that resulted from the improper disposal sited.
  • Imminent Health Hazard - Ceasing Operations & Reporting
    Observation: The permit holder failed to discontinue operations and notify the Health Department that an imminent health hazard may exist because of an emergency (sewage back up in basement).
