    Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health and activities if they are suspected of causing, or being exposed to a confirmed disease outbreak caused by Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus - employee did not know where the employee health policy was and the reporting procedure.
    Correction: Have the license holder require employees or applicants report to the person in charge if they meet one or more of the following high-risk conditions: Is suspected of causing, or being exposed to, a confirmed disease outbreak caused by S. Typhi, Shigella, Escherichia coli O157:H7, Hepatitis A virus or norovirus - review the Employee Health Policy with employees frequently.

    Observation: A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils or after engaging in any activity which may have contaminated his/her hands - food employee did not wash hands before donning new gloves.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food

    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: The comminuted pork for the McRib was not cooked to a sufficient temperature and time to eliminate pathogenic bacteria - the meat on part of the grill was not cooking properly.
    Correction: Continue to cook the meat to heat all parts to 155°F or above for 15 seconds, 145°F for 3 minutes, 150°F for 1 minute, or instantaneously at 158°F.

    Observation: @FOOD@ for which time rather than temperature is being used as a control was not labeled or otherwise marked with an updated discard by time.
    Correction: Discard food that is not cooked and served or served if ready-to-eat within 4 hours (or less hours or per written procedure) from the point in time when the food is removed from temperature control. Clearly label or otherwise mark the food with proper time frame for cook-serve or serve by time. Change the labels when there are new containers of food.

    Observation: The food contact surface of the @EQUIPMENT@ is not designed or constructed to be smooth and it contains cracks, chips, or pits that can not be easily cleaned - cracked food containersand lids.
    Correction: Replace the @EQUIPMENT@ to provide a food contact surface that is smooth and easily cleanable, free of breaks, open seams, cracks, chips, pits, sharp internal angles, corners, and crevices to prevent build-up of food debris.

    Observation: The lower drive-thru reach-in was observed in a state of disrepair and damaged - door will not keep closed.
    Correction: Repair the @EQUIPMENT@ to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: general cleaning needed.
    Correction: Maintain nonfood-contact surfaces of equipment clean.

    Observation: Food contact surfaces of the @EQUIPMENT@ used to prepare/store potentially hazardous food items were observed not being properly cleaned (washed, rinsed and sanitized).
    Correction: Clean food-contact surfaces of @EQUIPMENT@ no less than every 4 hours to prevent the growth microorganisms on those surfaces.

    Observation: Wiping cloths that were laundered on site were observed stacked wet in a laundry basket.
    Correction: Air-drying of cleaned linens must be done in a location that prevents contamination to both the cleaned linens and adjacent food preparation/storage areas. Relocate wiping cloths to allow contamination-free air-drying or enter into a contract with a linen service. This section does not apply if wiping cloths are stored after laundering in a sanitizing solution as specified under 12 VAC 5-421-1700.

    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles.     Correction: Relocate food storage to an approved area until the source of contamination is eliminated or removed. Buns were relocated.

    Observation: @POTENTIALLY HAZ FOOD@ noted not being adequately cooled to prevent the growth of harmful bacteria - one of the containers of sausage gravy.
    Correction: Cool potentially hazardous foods from 135°F to 70°F within 2 hours and within a total of 6 hours from 135°F to 41°F. A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses. The container of sausage gravy that was not cooling properly was disposed of.

    Observation: The following physical structures are not maintained in good repair: women's restroom - broken tile at handicapped toilet and grout in need of repair.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: The following physical structure was noted in need of cleaning: women's restroom floor.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
