Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

Observation: The prepared ready-to-eat (RTE) foods and the opened packages of RTE commercially processed foods in the refrigeration unit were not properly dated for disposition.
Correction: Mark the name and consume by or the disposition date on the containers of RTE foods and opened packages of commercially processed RTE foods of at the time of preparation if the foods are to be held under refrigeration for more than 24 hours.

Correction: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived foods and identification of the animal-derived foods by astericking (*) them to a footnote that states that the items are served raw or undercooked, or contain raw or undercooked ingredients.

Observation: A sanitizer test kit is not being used to ensure the chemical sanitizing solution(s) are at the proper concentration.
Correction: Depending on the sanitizer being use, obtain the correct chemical test kit to ensure the sanitizing solution is maintained at the proper solution.

Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the sink basin.
Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.

Observation: The dumpster had holes in the bottom that could allow rodents to get in.
Correction: Have the dumpster replaced with one that will keep rodents out.

Observation: The lid for the dumpster was damaged and could not cover the dumpster.
Correction: Have the lid secured/repaired so that it can cover the dumpster.

Observation: A sign or poster that notifies food employees to wash their hands is not provided at all hand sink used by food employees.

Observation: Mold growth was found on the wall by the back exit door.
Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
