Correction: Resurface/replace the cutting board (cannot be cleaned effectively).
Critical: Toxics - Common Name/working Containers of Toxics* (corrected on site) Observation: Observed a spray bottle of liquid on the wall rack with no labeling.
Correction: Label all toxic items.
This is a reinspection for the proper operability/temperature requirement of the tall reach-in refrigerator. The unit has been unplugged/awaiting a part.
Equipment - Good Repair and Proper Adjustment (repeated violation) Observation: Observed a torn refrigeration door gasket to the tall reach-in unit at end of cook line.
Correction: Replace the gasket.
Critical: Package Integrity* (corrected on site) Observation: Observed a badly dented can of tomato sauce on the rear can rack (can dated 2/12 on top).
Correction: Refrain from using badly dented cans of foods. Segregate dented cans from those to be used. Can was taken off rack.
Critical: Food - Potentially Hazardous Food - Cold Holding* Observation: Observed a bag of cheese/opened can of anchovies in the tall reach-in refrigerator at end of line at 56/57 degrees
Correction: should be 41 degrees or below. Observed the ambient air temperature of the tall reach-in unit was 57 degrees
Critical: Time as a Public Health Control* (corrected on site) Observation: Observed box of brownies/cookie in refrigeration unit at 57 degrees (boxes not product to remain frozen). Per employee policy deems that these are held for 4 hours.
Correction: If using time instead of temperature as a means of control, document times (time in/time to be discarded). Times supplied.
Equipment - Good Repair and Proper Adjustment Observation: Observed a torn refrigeration door gasket to the tall reach-in unit at end of cook line.

Critical: Toxics - Storage of Toxic Containers* (repeated violation) Observation: Observed 2 spray bottles of glass/oven cleaner stored adjacent to an open plastic container of flour.
Correction: Store toxic items separate from foods.
PIC knew symptoms associated with food borne illnesses. Permit not posted. Permit was found. Post in public view.
Equipment - Non-Food Contact Surfaces and Utensils (corrected on site) Observation: Can opener found dirty.
Permit can be reissued. New walk in has been installed which replaced two previous walk ins.
Critical: Eating, Drinking, or Using Tobacco* (corrected on site) Observation: The cook is drinking from an uncovered container in the food preparation area.
Correction: Employees may drink from a closed beverage container if the container is handled to prevent contamination of (1) The employee's hands, (2) The container, and (3) Exposed food, clean equipment, utensils, and linens, and unwrapped single-service and single-use articles. Drink was removed.
Food Storage - Clean and Dry Location Observation: Sodas stored beside and under handsink.
Equipment - Good Repair and Proper Adjustment Observation: Broken door handle to Turbo Air.
Correction: Repair. Pic stated this has been ordered.
Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils Observation: Mopsink found dirty.
PIC knew foodborne illness symptoms and 2 of 5 of the enteric diseases with which to exclude workers. Note: do not prop walk-in door open with a can of food. One walk-in is not working - plans to replace both walk-ins with one large walk-in. Sufficient light is required.
Critical: Ready-to-Eat - Potentially Hazardous Food - Disposition* (corrected on site) Observation: Chicken poppers dated 8/4-8/11. These exceeded 7 day shelf life.
Correction: PIC discarded. Remember 7 days include day of prep or opening therefore food opened on 8/4 would expire 8/10.
Critical: Sanitizer - Criteria/Chemicals for food contact* (corrected on site) Observation: Sanitizer in 3 compartment found at >500 ppm.
Correction: Sink was dumped. Also use 75 degree water instead of hot.
No permit posed. PIC called office for a duplicate. Good date marking system! Hood system looks good. Note: sauce has 10 hour time limit. Post no smoking sign in conspicuous location. Ok for permit.
Sanitizing Solutions, Testing Devices Observation: Could not find test strips.
Correction: PIC called for some to be delivered.
