Floors and general cleanliness of facility needs to be addressed. Syrup on floor behind the soda dispenser; sprinkles on floor. Pests observed, proof of pest control contract obtained. Ice scoop should be stored handle up in ice. Be sure all single service items are stored covered so pests cannot contaminate them. Facility cannot locate permit to display and needs another. Facility will change cheese dispensing operation to meet VDH regulatory requirements. Letter will be sent for person in charge to health department verifying the change of operation implementation.

Comments:
Complaint: Roaches were observed in facility on 12/28/06.
Investigation: At time of inspection no live roaches were observed. Dead roaches were however observed in traps. There are potentially multiple pest harborage areas. Some foods were observed on the floor as well that could be a contributing pest harborage factor. Though facility was being cleaned at time of inspection advised to fully clean facility to reduce risk of pest infestation. Owner provided pest control receipt with professional pest control provider as proof of pest control program being in place. Pest control provider comes every 30 days. Due to dead roaches being found in the facility the complaint has been confirmed.

PIC passed Demonstration of Knowledge Questions pertaining to this establishment (#1,2,3,5,9,12,13,17,4,6,7,18) Request a specific cleaning schedule be created and a copy submitted to VDH.
