  • Hands - Where to Wash (corrected on site)
    Observation: A food employee was observed cleaning their hands in the three compartment sink.
    Correction: Instruct food employees to clean their hands in a handwashing lavatory. Food employees may not clean their hands in a sink used for food preparation or utensil washing.
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (corrected on site)
    Observation: Raw beef observed stored next to unwrapped cabbage in the reach in cooler. Beef was placed in a stainless steel pan to prevent potential cross contamination of the cabbage.
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: The bean pupusa filling (46'F) and the pork pupusa filling (45'F) cold holding at improper temperatures. Foods were relocated to another cooler to cool.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.
  • Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking* (corrected on site) (repeated violation)
    Observation: The prepared ready-to-eat (RTE) foods, such as cooked chicken, cooked pork, pupusa filling and tomales in the refrigeration unit is not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
  • Single-Service and Single-Use Articles, Use Limitation
    Observation: Manufacturer container (originally used to store crushed red pepper) was observed reused for the storage of sesame seeds.
    Correction: Discontinue the reuse of manufacturer containers for food storage. Provide approved reusable food storage containers designed for your food storage needs.
  • Critical: Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils* (corrected on site)
    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: knife, can opener blade, kitchen scissors, and the wisk. PIC took all items to the three comparment sink to be cleaned.
    Correction: Clean and sanitize these surfaces for food contact.
  • Equipment - Non-Food Contact Surfaces and Utensils (repeated violation)
    Observation: Surfaces of the interior of the ice machine were observed soiled with accumulations of grime and debris.
    Correction: Clean the surfaces of the interior of the ice machine at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.
  • Non-Food Contact Surfaces
    Observation: The nonfood contact surfaces of the sides of the gas stove have accumulations of grime and debris.
    Correction: Clean the surfaces at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Employee Accommodations, Designated Areas
    Observation: Employee purses observed stored on dry storage shelving among food and single service equipment.
    Correction: Designate an area for employees to store their personal belongings where it will not contaminate food or equipment.
  • Physical Facilities in Good Repair
    Observation: Wall behind the mop sink is not maintained in good repair. Light bulb in the exhaust hood is noted burned out.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: Floor behind the gas stove is noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
  • Mops - Drying Mops (repeated violation)
    Observation: Mops not hung up to air dry.
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

Overall, facility observed well maintained and clean.
Manager/owner of facility was not available for the inspection, however employees were able to correctly answer most questions asked of them, as well as make corrections where necessary. A copy of the report was left with the employees, and EHS will return to facility to review report with the manager tomorrow afternoon.
Wiping cloth bucket measured 100 parts per million bleach water. Sanitizer in the three compartment sink measured between 100 and 200 parts per million bleach.

  • Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking* (corrected on site)
    Observation: The prepared ready-to-eat (RTE) foods such as pico de gallo and cooked meats in the refrigeration unit is not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: The can opener blade appears chipped and scratched, preventing proper operation and cleanability.
    Correction: Discard, replace or repair the can opener blade to provide proper operation, accuracy, functioning, maintenance, and cleanability.

    Observation: Surfaces of the interior of the ice machine were observed soiled with accumulations of grime and debris.
    Correction: Clean the surface of the interior of the ice machine at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.
  • Outer Openings - Protected
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents. Observed a small gap along the base of the rear door of the facility.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Handwashing Cleanser - Availability (corrected on site)
    Observation: Soap was not provided at the hand washing lavatory in the kitchen at the start of the inspection. PIC re-filled the soap dispenser.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: The floor drain at the three compartment sink is noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: Observed several live ants inside a bulk container of sugar. PIC agreed to discard all sugar from the bin and clean and sanitize the bin prior to use.
    Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required.

  • Critical: Hands - When to Wash* (corrected on site)
    Observation: A food employee failed to wash his or her hands before engaging in food preparation, after using a wet wiping cloth and before donning new gloves and returning to food preparation. The person in charge discussed this with the employee, and the employee washed her hands.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.
  • Handwashing - Using a Handwashing Lavatory - No Other Purpose (corrected on site)
    Observation: The handwash station in the food preparation area was observed being used to rinse wiping cloths.
    Correction: The handwash facility identified above is to be used for washing hands only
