Comments:
Follow up inspection coducted due to the activation of the sprinkler system above the stove on Saturday, 2.7.09 in the kitchen. Food supplies for the establishment is being temporarily held at the Sleep Inn Hotel, located one (1) block from the establishment. Walk in refrigerator is currently inoperable. The True refrigerator (glass door unit) is the only unit operable. No PHF on premises at this time for the daily continental breakfast.

Found the dishwashing machine fixed and water when wasted from the dishwashing machine did not drain onto the floor. All the cooking equipment were reasonably cleaned. More cleaning was needed where the mop sink and grease trap were located. Separation/identification of staff food inside the w-in cooler was still not done. Reminded Dawn that the Food Permit expires in August 2008 and that a Food Permit Renewal fee has increased from $40.00 to $100.00.

Comments:
Follow up on previous inspection. All critical violations have been corrected. Proper sanitizer test kit is now available. Other noncritical violations to be corrected as soon as possible. Waffle batter will be held at 41F during service period and at the end of the service period if any leftover is remaining it is to be discarded. Product will never be out longer than four hours. Usually product is out 15-20mins. However, a time as public health control policy is present. Permit to be issued upon recipient of application and payment.

Written procedures are maintained in the food establishment and made available to the regulatory authority upon request e. Provided info and discussed the following: 2007 Food code changes, cover letter from EH manager, operator health guide for exclusion and restriction of employee’s, 3 A’s of safe employee health practices, guide for employee’s to identify the top 4 reportable diseases and symptoms, emergency guidelines for food establishments, and the 2008 food handlers training schedule. Critical items 1770A and 3340 were corrected today.Please correct items 0850 and 2260 within 5 days. Other items should be corrected as soon as practical.

Have units checked (Walk in is holding at 42 F, marginal temperature ( I allow one degree for thermometer accuracy) PIC wrote up policy for discarding foods out of temp. Call me for follow up.
July 19, 2007 (Routine)
Violations: - 0530 - Yogurt containers are floating in water.
Use a container that will drain the melted water away from containers of yogurt. (A grate in bottom of bowl will work)
- 0820 - Critical 46 F cream cheese in glass door unit. Boiled eggs found at 69 F on buffet.
Have unit checked. It should hold foods at 41 F or below. Foods were relocated to walk in. Maintain eggs at 41 F or place on time control. (Manager wrote a time control policy during my visit to include these eggs). Discard eggs after today's breakfast.
- 1450 - Glass door unit is not operating at 41 F or below.
Have unit checked. A cup of water was placed inside unit. Tomorrow morning check water temp. Have serviced if needed.
- 2000 - Repeat Single service boxes on storeroom floor.
Elevate 6 inches.
- 2580 - A condensate drain line servicing A/C or 3 door refrigerator was emptying liquid waste directly onto the floor of the kitchen.
Repair/Relocate the condensate drain line to prevent liquid waste discharge onto the floor surface.
- 2930 - Screen is torn on window.
Repair so outer openings are protected.
- 3030 - No disposable towels were provided at the hand washing lavatory in the kitchen.
Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials Napkins were made available.
Comments:
Walk in has improved. Kitchen looks better organized. Call me for follow up so permit can be renewed.
December 14, 2006 (Routine)
Violations: - 1060 - Shelves have been lined with paper.
Remove paper , it is not cleanable.
- 1150 - Milk crates used as shelving.
Provide permanent shelving.
- 1530 - Repeat There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
Provide a chlorine test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
- 1570 - Repeat Door to chest freezer is in poor repair.
Door to be cleanable. Repair.
- 1770 C - Observed accumulations of dust, dirt, food residue or other debris on the following nonfood contact surfaces: shelf under grill..
Maintain nonfood-contact surfaces of equipment clean.
- 2000 - Box of single service bowls found on floor.
Elevate 6 inches.
- 2350 ii - Repeat Handsink drain is leaking. Pipe above dishmachine is dripping. Exterior reservoir from dishmachine is dripping.
Repair plumbing issues.
- 3170 - Repeat Ceiling panel missing above dishmachine. It appears walk in floor is rusty.
Replace ceiling panels and remove rust from floor.
- 3180 - Repeat Floors and walls inside walk in are dirty.
Clean.
Comments:
Note: It appears stopper to dishmachine does not always activate upon final rinse cycle. Call service person to repair. Ensure foods are cooled quickly in the WI (vent pans). Melons on breakfast bar are not kept. Tomato PIM and disaster brochure distributed.
August 29, 2006 (Routine)
Violations: - 0830 - Corrected During Inspection Critical The prepared ready-to-eat (RTE) cut melon in the refrigeration unit is not properly dated for disposition.
Mark the name and ""consume by"" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41F or below the food shall be served or sold within 7 calendar days or within 4 calendar days if held at 45F. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
- 1500 - The sliced bread at the breakfast line is not provided with dispensing utensil.
Provide separate dispensing utensils for each food item at the salad bar or buffet line to prevent cross contamination between foods.
- 1570 - Chest freezer has loose gasket and interior door panel is in poor repair.
Repair, replace or remove.
- 1750 - Repeat The bulk milk container tube was observed extending more than 1 inch from the chilled dispensing head. Tube was about 3 inches long.
The bulk milk container tube shall be cut on the diagonal to extend no more than 1 inch from the dispensing head to prevent contamination and to allow drainage of the tube.
- 1770 C - Observed accumulations of dust, dirt, food residue or other debris on the following nonfoodcontact surfaces: top of dishmachine.
Maintain nonfood-contact surfaces of equipment clean.
- 1790 - The cavity of the microwave oven is observed soiled.
Using the manufacturer's recommended cleaning procedure, clean the cavity of the microwave oven at least every 24 hours.
- 2350 ii - It appears there is a leak from fixture/line above dishmachine.
Repair
- 3170 - Repeat Walk in floor has rust. Holes in storeroom wall. Tile around mop sink is in poor repair.
Remove rust. Fill in holes so wall is smooth and cleanable. Replace missing/broken tile.
- 3340 - Corrected During Inspection Critical Container of insecticide is not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Can was stored beside cereal.
Containers of insecticide must be located in an area that is not above food, equipment, utensils, linens or single service items.
Comments:
Many improvements seen--extra light in walk in, new handsink faucet, storeroom is organized, holes covered in dining room wall. Permit issued.
June 30, 2006 (Routine)
Violations: - 0610 - Food stored on the floor or food stored less than 6" above the floor. (box of cheerios on storeroom floor)
Elevate food storage onto approved shelving with minimum 6" legs or casters.
- 0820 - Critical Butter found at room temperature (80 F).
Keep cold at 41 F or below. PIC discarded remaining packs.

- 3170 - Holes in wall where old coffee urns were installed.
Wall to be smooth and cleanable.
- 3180 - Walk in floor is dirty or rusty.
Clean thoroughly.
- 3330 - Critical Working containers of spray chemicals are not properly labeled.
Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents. Bottle was discarded.
Comments:
Note: If cut melon is kept longer than 24 hours you must mark with an expiration date of 7 days (include day of prep as day one). Towel on floor by 3 door refrigerator. PIC said it was a spill. Organization needed throughout kitchen and storeroom. Discard food not kept at proper temperatures as per written policy.
February 17, 2006 (Follow-up)
Comments:
Dishmachine is feeding sanitizer properly but stopper isn't closing by itself during the final rinse causing sanitizer and drying agent to run out the drain. Call service person to check.

It appears dishmachine has not been serviced. PIC could not prime unit. Paper goods are being used. I asked her to call me when unit has been repaired.

Violations: - 0220 - Critical Personnel is drinking from an uncovered container in the food preparation area.
Employees may drink from a closed beverage container with straw if the container is handled to prevent contamination of (1) The employee's hands, (2) The container, and (3) Exposed food, clean equipment, utensils, and linens, and unwrapped single-service and single-use articles.
- 0470 - Corrected During Inspection Critical Raw animal food stored over ready-to-eat (RTE) food in the refrigeration unit. Whole shell eggs over prepared foods. PIC rearranged food.
Separate raw animal foods during storage, preparation, holding, and display from raw RTE food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
- 0820 - Corrected During Inspection Critical Butter packs found at room temp.
Maintain at 41 F or below. PIC discarded butter.
- 0850 - Critical No written procedures for the use of time as a public health control with potentially hazardous foods.
Provide written procedures for the use of time as a public health control. The procedures must include the following steps or practices: a. Food must be marked with the time within which it must be cooked, served or discarded. b. The food is served or discarded within four hours from the time when the food is removed from temperature control c. Food in unmarked containers or packages, or for which the time expires is discarded and d. Written procedures are maintained in the food establishment and made available to the regulatory authority upon request e. Once time is implemented as a control measure for potentially hazardous food, no other measures may be substituted.

If they are not cleanable you must replace.
Comments:
Refrigeration units appear to be in working order. Dishmachine is operating properly. Above items to be corrected by new owner. Officer of corporation is to sign application. Please return copy to our office with correct signature. Permit issued.
