    Place a protective barrier in between plates if you are going to stack plates.
  • 1770 A - Critical Observed 2 fountain nozzles at the bar that need cleaning (green substance).
    Clean the nozzles more frequently/thoroughly.

Comments:
Investigation of complaint alleging that on 6-20-06 at 1-2 pm customer found hair in pasta dish and fly in bottom of shrimp scampi dish. Spoke with manager who was aware of the complaint. Everyone in the kitchen has hair restraints on and routinely wears them. Observed a fly in the bar, none in dining room or kitchen. Pest control done by professional PCO.

November 10, 2004 (Complaint)

Comments:
Investigation of complaint alleging that on 11/9/04 complainant served salad with bug with wings in it. Also entree/shrimp had long red hair in it. Spoke with manager who was working at time of complaint and did see bug-looked like gnat-very small fly like bug in lettuce in salad. Lettuce is received bagged, prewashed-placed into hopper and refrigerated until use-at end of night remaining lettuce is discarded. Hoppers are washed and sanitized between uses. No flies observed today. Source of hair undetermined. Server had dyed red hair that was pulled back into pony tail. Employees have hair restrained today.

  • 3170 - Floor in walk in freezer is damaged. Grout in floor by dish wash area is eroded and needs to be replaced.
