Violation: 1580 - The cutting board(s) in the prep area are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
Comments:
WIC-walk in cooler, RIC-reach in cooler, ST-surface temperature, IT-internal temperature, HH-hot holding, CH-cold holding, PIC-person in charge, SP-sandwich prep, RIR-reach in refrigerator
No food prep going on during inspection. Halfway house next door is not in use at this time. Food supplier is US Foods. Discussed with operator: 1. Ensure black trash bags are not used to store prepackaged food items in bulk food containers. 2. Carbon build up on sheet pans is removed at least twice a year. 3. Ensure thermometer for taking food temperatures is calibrated on a regular basis. Facility is very old but very well maintained and organized. Observed good cold holding temperatures.

Comments:
Food is no longer permitted in the halfway house which is affiliated with the treatment house. No food prep during inspection. Facility will be relocating to another site within approximately the next 8 months. Advised operator, that plans will need to be submitted to the Health Department for new facility or new remodeled facility, 30 working days prior to relocation. Ensure refrigeration units are set at 36'F to 38'F in order to keep all potentially hazardous foods at 41'F or lower.

  • 1570 - Unused or non-functioning microwave in the halfway house not removed from the premises.
    Remove any unused or non-functioning equipment from the premises.
  • 1570 - Bottom of cabinet below the sink in the halfway house was observed in a condition that prevents necessary maintenance and easy cleaning.
    Repair the microwave to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the EQUIPMENT, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • 2640 - The covers of the refuse containers located outside the establishment are not a tight fit. Covers are damaged.
    Replace the door of the outside refuse container with one that is a tight fit.
  • 2930 - Outer opening of the food establishment is not protected against entry of insects and rodents. Observed holes in screen in open window above the 3 compartment sink.
    Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • 3350 - Corrected During Inspection Critical Unnecessary poisonous or toxic materials found in the food establishment. Observed can of Bug Stop in basement storage room in treatment house.
    Remove unnecessary poisonous or toxic materials.
Comments:
Also inspected halfway house (521 Sophia Street) which is affiliated with the treatment house. There are no cooking facilities in this house. Residents make their lunch in this house - sandwiches only. Kitchen facilities are in the treatment house next door to halfway house.
Facility does not prepare roasts and/or undercooked fish.

Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.

Prepared Foodservice Establishment Evaluation Report & Permit, spoke with PIC regarding report & permit, and faxed both to PIC.

Discussed with PIC: RI Refrigerator in the kitchen area should be set at around 38'F to ensure that food is kept @ 41'F or colder, light fixture in the kitchen area is to be kept clean, and containers of sugar and flour are to be labeled.
