Suggested air drying rubber floor mats to prevent unnecessary water and grime accumulations over night. Suggested removing waste immediately when waste product is a pest attractant. Observed food handler cards, employee health policy, and temperature charts. CFM certificate needs to be displayed in public view. Approved for permit.
  • Critical: Equipment and Utensils - Hot Water and Chemical Sanitizing* (corrected on site)
    Observation: When tested, no sanitizer or a low concentration of chemical sanitizer was found in the wiping cloth bucket.
    Correction: Provide Bleach at proper concentration of approximately 100 ppm and immerse or expose food contact surfaces to sanitizing solution for adequate time.

    Observation: The person in charge failed at being a certified food protection manager who has shown proficiency of required information through passing a test that is part of an accredited program or responds correctly to the EHS's questions as they relate to the specific food operation.
    Correction: Ensure that the person in charge has a valid certified food protection manager certificate and/or correctly demonstrates knowledge of the food operation as it relates to the EHS's questions.
  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: Atmospheric breaker on mop sink was observed in a state of disrepair and damaged.
    Correction: Repair the atmospheric breaker to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the breaker, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health and activities if they are suspected of causing, or being exposed to a confirmed disease outbreak caused by Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus.
    Correction: Have the license holder require employees or applicants report to the person in charge if they meet one or more of the following high-risk conditions: Is suspected of causing, or being exposed to, a confirmed disease outbreak caused by S. Typhi, Shigella spp., Escherichia colo 0157:H7, Hepatitis A virus or Norovirus.
  • Sanitizing Solutions, Testing Devices
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a bleach test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Equipment - Good Repair and Proper Adjustment

    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.
  • Critical: Toxics - Storage of Toxic Containers* (corrected on site)
    Observation: Chemicals are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Observed chemicals stored next to single use items (paper towels).
    Correction: Containers of chemicals must be located in an area that is not above food, equipment, utensils, linens or single service items.

Provided application, food handler class schedule, new employee health policy, effective changes to 2013 FDA Food Code, and Certified Food Manager class schedule. Approved for permit renewal.

    Observation: Foods not properly labelled. Observed storage container for various sauces not properly labelled.
    Correction: Foods, whether packaged from the provider,packed within the establishment,or contained in working containers are required to conform to specific labeling laws.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: the reach in dairy refrigerator in the back room. Spoiled milk was observed on the lowest shelf of the unit.
    Correction: Maintain nonfood-contact surfaces of equipment clean.
  • Backflow Prevention Device, Design Standard
    Observation: The backflow or backsiphonage prevention device installed on the mop sink is observed in disrepair and is no longer meeting the standards set by the American Society of Sanitary Engineering.
    Correction: Provide the approved backflow or backsiphonage device for the specific application and type of device to prevent contamination of the drinking water system.
