No violations noted at time of inspection. Discussed employee health policy with operator and provided FDA form 1B. Facility is to have an policy in place by the time of the next scheduled inspection. If any foods are to be served raw and/or undercooked (i.e. hamburgers) the facility must have a consumer advisory alerting customers to the increased risk of consuming those foods. An example of the policy would be: "Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness." Any advisory must be clearly posted on menus or on display board and must be linked to foods which could be served raw and/or undercooked by some unique symbol (asterisk, star, etc.).

1800 - The nonfood contact surface of the fan has accumulations of grime and debris. Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
2650 - There is no refuse container at the area immediately adjacent to the prep hand sink. Provide a refuse container for the disposal of paper towels at the above hand sink.
