Rusting metal shelving in the Walk In Refrigerator are covered by a laminate floor covering (not acceptable). Correct rusting shelving and removed unapproved laminate floor covering used to cover rusting shelves. HEALTH DEPARTMENT INSPECTION RECORDS ARE NOT BEING MAINTAINED ON THE PREMISES FOR A PERIOD OF ONE YEAR AS DIRECTED (CORRECT IMMEDIATELY).
